Transitional Care Registered Nurse (RN)

We’re hiring a Transitional Care Registered Nurse (RN) in Atlanta, Georgia who wants a stable weekday schedule, hospital-based practice, and meaningful hospice work.

You’ll step into a clinical hospital-based role focused on coordinating hospice admissions and managing General Inpatient (GIP) patients. This is a stationary position primarily based at a major Atlanta hospital, with occasional coverage at a second nearby facility. You’ll work closely with physicians, hospital staff, and the hospice interdisciplinary team to ensure seamless transitions of care.

• Base salary: $90,000–$95,000
