    Position Summary

    The Transportation Coordinator is responsible for planning, scheduling, and overseeing all transportation services for the adult day center to ensure safe, efficient, and reliable transport of participants. This role coordinates daily routes, assigns drivers, and ensures transportation services are delivered in accordance with participant care plans and all applicable Ohio regulations, including Department of Aging, Centers for Medicare & Medicaid Services (CMS), and Medicaid Non-Medical Transportation (NMT) requirements.

    The Transportation Coordinator serves as the central point of communication between participants, families, drivers, and program staff, ensuring timely service delivery, accurate documentation, and a high standard of participant safety, dignity, and satisfaction.

    Duties and Responsibilities:

    Develop and manage daily, weekly, and recurring transportation schedules

    Optimize routes for efficiency while meeting participant needs and service timeframes

    Assign drivers and vehicles based on participant requirements (e.g., wheelchair access, staffing needs)

    Adjust schedules in real-time to accommodate call-offs, delays, or changes in participant status

    Supervise transportation staff, including drivers and aides (if applicable)

    Provide direction, coaching, and support to ensure safe and professional service delivery

    Coordinate driver schedules, timekeeping, and coverage for absences

    Ensure all drivers maintain required certifications (e.g., CPR/First Aid, defensive driving)

    Communicate with participants, caregivers, and families regarding pick-up/drop-off times and changes

    Ensure transportation aligns with participant care plans, mobility needs, and behavioral supports

    Address and resolve transportation concerns, complaints, or service issues promptly

    Ensure all transportation services comply with the Ohio Department of Aging, CMS, and Medicaid NMT requirements

    Maintain accurate records, including schedules, trip logs, mileage, incident reports, and billing documentation

    Support audit readiness by organizing and maintaining required transportation records

    Ensure adherence to HIPAA and confidentiality standards

    Monitor completion of pre-trip and post-trip vehicle inspections

    Coordinate routine vehicle maintenance, repairs, and inspections

    Ensure all vehicles meet safety standards and are equipped with required emergency supplies

    Investigate and document accidents, incidents, or safety concerns

    Serve as liaison between transportation staff and program/clinical teams

    Communicate daily transportation updates to internal staff

    Collaborate with leadership on service improvements, cost efficiency, and capacity planning

    Respond to transportation-related emergencies, including vehicle breakdowns or accidents

    Implement contingency plans to ensure continuity of services

    Complete incident reporting and follow-up as required

    Follows all center policies and procedures

    Required Qualifications:

    High school diploma or GED.

    Must have effective written and verbal communication skills.

    Effective communication with all staff, participants, caregivers, and/or families.

    Must have good organizational and time management skills are essential.

    Clean Driving record/abstract.

    Valid Driver’s License and insurance.

    Passenger Assistance Safety and Sensitivity Training (PASS) and/or DRIVE Training (provided by Care Forever).

    Preferred Qualifications:

    Minimum of 2 years of experience as a Driver licensed in the state of Ohio

    Experience in driving larger, 12+ passenger, and accessibility enabled vehicles.

    Bi/multilingual.

    Physical Demands:

    Ability to sit, stand, walk, bend, and squat for prolonged periods; ability to push and pull objects and lift and carry up to 20 pounds, unassisted, on a frequent basis and occasionally up to 50 pounds. Full use of gross and fine motor skills of the upper and lower extremities (i.e., writing in charts, assisting with toileting and incontinence care, assisting with meals, activities, etc.)

    Working Conditions:

    Primarily inside a well-lit, well- ventilated area. Moderate time inside in a vehicle. Moderate periods of outside walking and assisting participants to and from the vehicle.
