    Position Summary

    The Transportation Professional (TP) is responsible for the safe and timely transportation of participants to and from the adult day program, medical appointments, and community-based activities. This role ensures participant safety, dignity, and comfort while assisting with boarding, securement, and supervision during transport.

    The TP serves as a key member of the care team by observing and reporting changes in participants' conditions, maintaining accurate transportation documentation, and ensuring that all services are delivered in compliance with the Ohio Department of Aging, CMS, and applicable Medicaid transportation regulations.

    Duties and Responsibilities:

    Provide safe and reliable transportation to participants of the center by following all state and federal requirements for non-medical transportation.

    Assistance with loading and unloading of the participants, including but not limited to assisting in getting in and out of the vehicle, fastening seatbelts, lap belts, and shoulder belts, assisting participants in the vehicle who use wheelchairs, and securing wheelchairs in the vehicle.

    Assist participants who need door-to-door service.

    Maintain supervision levels while in the vehicle and assist participants in loading and unloading.

    Manage a wheelchair, safely assist participant with transfers, operate wheelchair lift, and use adaptive equipment.

    Complete required documentation such as pre/post vehicle inspections, transportation logs, and incident reports.

    Communicate any safety issues with the vehicle immediately to the Program Director.

    First Aid and CPR Certified.

    Valid driver’s license for the state the center operates in.

    Will refrain from distracted driving and driving under the influence of drugs (including prescription medications) and alcohol.

    Keep the vehicles clean inside and out. Fuel the vehicle when it reaches ¼ tank of gas.

    Report any changes in participants’ conditions to the Program Director, Nurse, or Transportation Coordinator

    Employee must maintain effective communication with all staff, participants, caregivers, and/or families.

    Completes continuing education as required and maintains any certification essential to the position.

    Follows the center’s policies and procedures.

    Responsible for facilitating day-to-day activities and leisure and recreational programs of participants in groups and individually, to help meet their social, spiritual, cognitive, physical, and psychosocial needs.

    Responsible for assisting participants to meet their functional needs in the areas of dressing, grooming, eating, continence, transferring, and toileting, while supporting the participants' desired level of independence.

    Must maintain participant confidentiality at all times.

    Required Qualifications:

    High school diploma or GED.

    Must have effective written and verbal communication skills.

    Effective communication with all staff, participants, caregivers, and/or families.

    Must have good organizational and time management skills are essential.

    Must have a clean driving record, a valid Driver’s License, and automotive insurance

    Passenger Assistance Safety and Sensitivity Training (PASS) and/or DRIVE Training (provided by Care Forever)

    Preferred Qualifications:

    Minimum of 1 year of experience in transportation services.

    Minimum of 1 year of experience in long-term care (residential, adult day services, community-based care) or dementia care.

    EMT Certification

    Physical Demands:

    Ability to sit, stand, walk, bend, and squat for prolonged periods; ability to push and pull objects and lift and carry up to 20 pounds, unassisted, on a frequent basis and occasionally up to 50 pounds. Full use of gross and fine motor skills of the upper and lower extremities (i.e., writing in charts, assisting with toileting and incontinence care, assisting with meals, activities, etc.) Visual acuity to drive the company vehicle safely.

    Working Conditions:

    Primarily driving the company vehicle in all weather conditions up to 8 hours a day. Secondary inside in well-lit, well-ventilated areas up to 3 hours a day.
