Our client is seeking a skilled Inpatient Clinical Pharmacist to join their hospital pharmacy team. This role offers the opportunity to make a meaningful impact on patient outcomes through direct pharmaceutical care, clinical collaboration, and expert medication management in an inpatient setting.
Key Responsibilities
- Provide direct pharmaceutical care to hospitalized patients, ensuring safe, effective, and evidence-based medication use.
- Review and verify physician medication orders for accuracy, dosage, and clinical appropriateness.
- Participate in daily medical rounds and collaborate with physicians, nurses, and other healthcare professionals to optimize therapy plans.
- Monitor patient progress and adjust medication regimens based on lab results and clinical outcomes.
- Prepare and dispense sterile and non-sterile compounded medications in accordance with regulatory and hospital standards.
- Identify, prevent, and resolve adverse drug reactions, therapeutic duplications, and drug interactions.
- Document clinical activities, interventions, and patient education in the medical record.
- Provide counseling to patients and families on medication use prior to discharge.
- Support formulary management, antimicrobial stewardship, and quality assurance initiatives.
- Contribute to ongoing process improvement and patient safety programs.
Qualifications
- Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.
- Active pharmacist license in good standing
- BLS certification required; ACLS certification within 1 year of hire.
- Hospital or clinical pharmacy experience preferred; PGY-1 residency is a plus.
- Must successfully pass a background investigation and meet requirements under the Indian Child Protection and Family Violence Prevention Act (ICPFVP).
- Valid, unrestricted, and insurable driver’s license.
- Bilingual skills in English and Navajo preferred.
Knowledge & Skills
- Comprehensive understanding of pharmacotherapy, aseptic technique, and inpatient medication systems.
- Ability to interpret clinical data, lab values, and vital signs to guide therapy.
- Strong collaboration and communication skills within a multidisciplinary team environment.
