  • Responsibilities

    Our client is seeking a skilled Inpatient Clinical Pharmacist to join their hospital pharmacy team. This role offers the opportunity to make a meaningful impact on patient outcomes through direct pharmaceutical care, clinical collaboration, and expert medication management in an inpatient setting.

    Key Responsibilities

    • Provide direct pharmaceutical care to hospitalized patients, ensuring safe, effective, and evidence-based medication use.
    • Review and verify physician medication orders for accuracy, dosage, and clinical appropriateness.
    • Participate in daily medical rounds and collaborate with physicians, nurses, and other healthcare professionals to optimize therapy plans.
    • Monitor patient progress and adjust medication regimens based on lab results and clinical outcomes.
    • Prepare and dispense sterile and non-sterile compounded medications in accordance with regulatory and hospital standards.
    • Identify, prevent, and resolve adverse drug reactions, therapeutic duplications, and drug interactions.
    • Document clinical activities, interventions, and patient education in the medical record.
    • Provide counseling to patients and families on medication use prior to discharge.
    • Support formulary management, antimicrobial stewardship, and quality assurance initiatives.
    • Contribute to ongoing process improvement and patient safety programs.

    Qualifications

    • Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.
    • Active pharmacist license in good standing
    • BLS certification required; ACLS certification within 1 year of hire.
    • Hospital or clinical pharmacy experience preferred; PGY-1 residency is a plus.
    • Must successfully pass a background investigation and meet requirements under the Indian Child Protection and Family Violence Prevention Act (ICPFVP).
    • Valid, unrestricted, and insurable driver’s license.
    • Bilingual skills in English and Navajo preferred.

    Knowledge & Skills

    • Comprehensive understanding of pharmacotherapy, aseptic technique, and inpatient medication systems.
    • Ability to interpret clinical data, lab values, and vital signs to guide therapy.
    • Strong collaboration and communication skills within a multidisciplinary team environment.
    • Detail-oriented with strong critical thinking and problem-solving skills.
    • Commitment to confidentiality, ethics, and excellence in patient care.
