Position Overview

We are seeking a highly skilled, compassionate, and experienced Licensed Physical Therapist (PT) to provide in-home care for a patient recovering from a recent stroke. The primary focus of therapy will be neurological rehabilitation with an emphasis on mobility, gait retraining, and strengthening exercises to support the patient’s independence and quality of life.

This is a travel position in a beautiful, rural home setting in Big Sur, CA, approximately 10 minutes from Highway 1. The ideal candidate will have strong neuro PT experience, excellent communication skills, and a calm, patient-centered approach.

Key Responsibilities

Conduct individualized physical therapy sessions focused on stroke recovery and functional mobility

Develop and implement personalized care plans based on patient progress and goals

Provide gait training, balance exercises, strength training, and range-of-motion therapy

Collaborate with family members and caregivers to reinforce home exercise programs

Track and document progress, updating care plans as needed

Communicate clearly and compassionately with the patient and family

Qualifications

Current California Physical Therapist License (DPT or MPT required)

Minimum 3 years of clinical experience, with strong preference for neurological rehabilitation or stroke recovery

Experience with in-home care or home health PT preferred

Strong clinical judgment and ability to work independently

Excellent verbal, written, and interpersonal communication skills

Reliable transportation and ability to travel to remote areas of Big Sur (4WD or AWD vehicle preferred)

Current CPR/BLS certification

Preferred Skills

Certification in Neurologic Physical Therapy (NCS) or specialized training in neuro rehab techniques (e.g., PNF, NDT, CIMT)

Experience working with older adults and post-stroke patients

Comfort navigating rural terrain and providing care in non-traditional clinical settings

Ability to adjust treatment plans creatively and compassionately based on patient feedback and progress
