    INTENSIVE CARE CASE MANAGER - CHAMPION PLAN

    Gandara Center is a member of the City of Brockton Mayor’s Champion Plan collaboration and Brockton Neighborhood Health Center’s RIZE collaboration formed to address the local challenges associated with the opioid epidemic. Collaborations include Brockton Police Department, representatives from Plymouth County Correctional Facility, Probation Department, emergency medical services, local treatment providers and City of Brockton Social Services Department.

    The INTENSIVE CARE CASE MANAGER - CHAMPION PLAN we are looking for in this position will work as a member of an interagency outreach team with specific training and orientation related to substance use, recovery support services, opioid overdose prevention and harm reduction, as well as linkage to on-going recovery supports and treatment. Duties and responsibilities include outreach and contact coordination, overdose prevention education, provision of support services and referrals to primary/specialty and behavioral health care, recovery support services and social services.

    DUTIES AND RESPONSIBILITIES:

    • Duties and Responsibilities:
    • Navigation and support to appropriate services for people at risk of opioid overdose.
    • Provide support in accessing Naloxone therapy and overdose prevention education.
    • Outreach, with the BAMSI CP Prevention Educator, during morning shifts to streets and locations identified as high-risk sites through mapping of overdose locations.
    • Work collaboratively with the Brockton Police Department and Plymouth County Outreach (PCO).
    • Deliver individual recovery supports, relevant overdose prevention education and materials/service referrals, promotion/prevention and harm reduction education.
    • Assist clients with needed paperwork such as applications for birth certificates, behavioral and medical health appointments, and support applications such as benefits and housing, as required.
    • Accompany clients to medical appointments may be required, to provide emotional support.
    • Assist clients to develop their capacities around health promotion/treatment/medical scheduling and health management planning, taking into consideration the reality of the relevant social determinants of health for everyone.
    • May provide first contact support for other health conditions (e.g., infectious diseases, etc.) offering testing options at Outreach locations, through health network partners.
    • Attend all required training sessions, inter-agency team meetings, staff meetings and supervision sessions.
    • Collect required client data, maintain accurate and complete contact and case management
    • notes/records.
    • Maintain client confidentiality, follow codes of conduct and ethics related to professional behavior and boundaries.
    • Work as a team member in a diverse and multi-disciplinary team.

     

     

    Required Skills

    QUALIFICATION FOR POSITION:

     

    • Must have at least 5 years’ previous experience in Mental Health or Substance Abuse service system. Some administrative experience preferred.
    • Bachelor’s degree required
    • Certified Recovery Coach/LADC a plus. Must be willing to obtain Recovery Coach Certification within 6 months of hire
    • Must have a valid drivers’ license and a vehicle.

     

     

    SUPERVISORY RELATIONSHIPS:

    Works under the supervision of the program director. Required to function independently, but he/she must provide regular updates and status reports regarding contact with clients in the Program.

     

    SKILLS, KNOWLEDGE AND PRIOR EXPERIENCE REQUIRED:

    Experience with case management, outreach and recovery support. Strong community relationships in the region including knowledge of police and court diversion projects, service providers for linkages to housing, vocational and educational opportunities, health insurance and primary care and dental services, MAT providers, and other support services as needed/directed by the participant. People in recovery from substance use and who have demonstrated a history of peer-to-peer supports are encouraged to apply. Bi-lingual candidates are encouraged to apply.
