Qualifications: The psychiatric nurse practitioner must be a licensed nurse practitioner with experience with the population served.

    The primary purpose of this position is to provide clinical services to all clients receiving ACT team services, which includes medication prescription, administration, monitoring, and documentation; directing psychopharmacologic and medical services; service coordination, including working with the ACT team to monitor each individual’s clinical status and response to treatment; crisis assessment and intervention; symptom assessment and management; individual counseling and psychotherapy; substance use treatment; work-related services; activities of daily living services; social, interpersonal relationship, and leisure-time activity services; support services or direct assistance to ensure that individuals obtain the basic necessities of daily life; and education, support, and consultation to individuals’ families and other major supports.

    We are seeking a part-time Nurse Practitioner to provide the following services:

    • Supervise all medical/clinical aspects of the services delivered.
    • Identify appropriate evidence-based or emerging best practices therapies to be employed by staff in delivering services.
    • Consult with primary healthcare providers/other providers on the clinical aspects of the clients being served and the implications for those clients’ physical healthcare needs.
    • Coordinate with medical staff of other providers, e.g. state and community inpatient facilities.
      Supervise all other medical staff on the ACT team.
    • Assist in identifying, planning, scheduling, and implementing services for the client.
    • Crisis phone/on-call duties are rotated amongst team members.
    • Assist in the development of transition plans when clients move in and out of services.
    • Provide written documentation as required.
    • This is considered Light Work; no lifting over 20 pounds.
    • Must be free from all illicit substance use.
    • Demonstrate fidelity to ACT Model as measured by TMACT.
    • At the discretion of team lead, supervisor, and/or management, this position may require additional duties not specifically listed above.
