    The primary purpose of this position is to provide Community Support Team Services to Mental Health and Substance Abuse consumers. Job duties include but are not limited to; supportive counseling, psycho-educational activities, behavioral interventions, training to caregivers/family/natural supports, vocational support, educational support, rehabilitative and independent living skills, community integration, skill building including daily & community living skills; socialization skills, adaptation skills, development of leisure time interests/activities; symptom management skills; wellness education; substance abuse education; work readiness and any and all areas identified as needed by the client’s individual Person-Centered Plan.

    We are seeking a full-time Associate Professional to provide the following services:

    List of Responsibilities, Requirements, and Duties

    • Serve as a member of the client’s Community Support Team in the implementation of the consumer’s Person-Centered Plan.
    • Assist in planning, scheduling and implementation of services for the clients.
    • Work to implement service delivery with other staff and/or across agencies.
    • Develop therapeutic interventions to work with consumer.
    • Provide any written documentation as required. This will include treatment notes, goals, and plans for the medical record (e.g., Service notes, input into person-centered plan, etc.)
    • Be available to respond as needed to clients and co-workers 24/7/365.
    • Must be free from all illicit substance use.
    • This is considered Light Work; no lifting over 20 pounds.

    Direct Services

    • Work to implement behavioral interventions, crisis interventions and rehabilitative and psycho-educational activities.
    • Work with clients in reviewing, applying and achieving their goals as indicated on the Person-Centered Plan.
    • Assist and/or coordinate as assigned in planning and implementing community engagement and other such activities as related to the client’s therapeutic goals and
    • mental health needs.
    • Utilize community-based activities as a means to deliver clinically appropriate services.
    • Assist other clinical staff in parent/guardian/natural support training that will aid them in dealing more effectively with the client.
    • Assist in all service delivery as formally planned with the clinical team.
