    We are seeking a full-time Peer Support Specialist for the Community Transition Recovery Program to increase an individual’s ability to live successfully in the community and achieve their goals while residing in the program. Services provided include, but are not limited to problem-solving, coaching, skill training around daily hygiene and grooming, grocery shopping, cleaning, laundry, and taking medications as prescribed.

These services include evidence-based practices, school-based and outpatient therapy, psychiatric evaluations and treatment, Community Support and substance abuse services to name a few.

    We are seeking a full-time Peer Support Specialist for the Community Transition Recovery Program to provide the following services:

    • Assists and supports activities-of-daily-living, social, interpersonal relationship, and leisure-time-activity services;
    • Educates, supports, and consults with individuals’ families and other major supports;
    • Actively explore and pursue Community Inclusion opportunities with individuals;
    • Actively explore employment and education options with individuals, with emphasis on IPS-Supported Employment;
    • Actively search for safe and affordable housing;
    • Assist and support individuals with ability to inhabit new housing upon move in;
    • Assist and support individuals with shopping for items needed to maintain community living;
    • Assist and support individuals when they separate from housing (e.g. move out furniture, secure storage, work with natural supports, etc.);
    • Assist and support individuals with re-housing plans;
    • Assist and support individuals with completion of FL-2 and applying for Special Assistance (SA) In-home;
    • Foster individual’s development of healthy relationships by encouraging participation in activities and providing services within the community primarily face-to-face;
    • Completes treatment notes, goals, and plans for the medical record (e.g., service notes, input into person-centered plan, etc.)
