__Job Description

Nurses will be working in a home health environment, providing 1:1 continuous care with hospice care patients in end of life situations in a patient's home, providing medication support or any type of medical support to assist in stabilizing patients until end of life. Shifts will be 12 hours each rotating with other nurses

Job Qualifications

• Graduate of accredited practical/vocational nursing program, Associate degree preferred
• Currently Licensed Practical/Vocational Nurse in state(s) of California,
• Current (CPR) certification
• Minimum of 1 year of experience in home health hospice or palliative care, comfortable dealing with end of life patient care
• Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order

Schedule

• On Call
• AM/PM shifts based on 12 hr shifts (6am-6pm , 6pm-6am)
• Times and days subject to change based on patient needs
