    POSITION: RN Clinical Manager - Home Health
     
    LOCATION: Anaheim, CA, US
     
    SCHEDULE: Full Time, 40 Hours per Week
     
    SHIFTS: Day shifts
     
    PAY: $42–$70/Hr.
     
    OTHER: $10,000 SIGN-ON BONUS POTENTIAL (for external candidates only)!

    ESSENTIAL DUTIES & RESPONSIBILITIES: include, but not limited to, the following:
    • Ensure optimal staff levels to meet safe and timely patient care delivery
    • Manage resources, productivity, and third party reimbursement guidelines and methods to meet the annual operating budget
    • Effective problem-solving skills and follow through on patient care issues, including notification of appropriate personnel, and/or physician
    • Collaborate with the Director of Home Health as needed
    • Provides ongoing guidance and support to clinical field and office staff to ensure patient needs are met according to the care plan; requires the home health manager to be available for guidance and support to clinical staff according to the time of services being provided.
    • Manage clinical team members, including nurses, social workers, home health aides, and office staff
    • Assess competence of staff, and compliance with Standard Precautions, and quality of care delivery
    • Promptly and fully investigate and resolve complaints of patients, families, physicians, team members, etc.
    • Provide timely feedback to direct reports, including addressing performance weaknesses and coaching staff for improvement.
    • Oversee and assist with coordination and continuum of care
    • Manage team schedules
    • Oversee patient initial and ongoing Plan of Treatment, recertification and discharge care, and documentation to ensure compliance with agency policy, law, and regulations
    • Complete joint supervisory home visits to ensure compliance with network policies
    • Collaborates with QA/PI staff to provide completion of specific management reports for timeliness of certifications, recertification’s, and compliance with managed care authorizations
    • Participate in agency Improving Organizational Performance activities
    • Oversee completion of Compliance Education
    • Knowledgeable in policies, regulations, and governing body requirements
     
    QUALIFICATIONS: The minimum academic and relevant experience include the following:
    • Bachelor’s degree in Nursing (BSN) or related field. RNs without a BSN must obtain a BSN or a degree in a related field within 6 years of hire.
    • Minimum of 1-year Leadership/supervisory experience
    • At least two years Clinical experience within the last 5 years in home health primary care clinic or health facility
    • Current California RN license.
    • CA Driver’s License.
    • Proof of auto insurance.
    • Current BLS
    • Possess good management skills such as effective verbal and written communication, prioritization, time management, organization, and critical thinking
    • Good understanding of computer software and can easily adapt to computer programs
    • Ability to follow Agency's policies and applicable laws
    • Knowledgeable of federal Conditions of Participation, state licensing regulations and accreditation standards, and ability to interpret regulations for clinical staff to ensure patient advocacy, informed decision making, and compliance with policies and regulations
    • Knowledgeable of OASIS, basic coding, and evidence-based care standards
    • Knowledge of human resource principles and network policy
    • Demonstrate current knowledge in the clinical area
    • Demonstrate Values-based customer service skills
