    POSITION SUMMARY:

    We're looking for a Nurse Care Coordinator (NCC) to join our Clinical Care Team. You'll work with our cross-functional team, interfacing with the internal UCM Team and external vendors about innovative ways to help navigate challenges that may stand in the way of the patients needs. You'll get to know patients through video/phone conversations along with their medical record, provide education and suggest appropriate treatments based on our comprehensive approach to maximize health. You'll be supported by physicians' assistants, nurse practitioners, physicians and a care coordination team to ensure patients get the care they need and remain engaged in that care. You'll be reporting to the VP, Clinical Operations.

    The ideal candidate has a strong understanding of emergency and urgent care, primary care and mental health, experience in clinical case management, has an interest in using technology to deliver better, more personalized, convenient care and works well independently as well as a member of a team. You have extensive experience with patients and enjoy the process of guiding and advocating for patients during their care journey with their best interest in mind. Lastly, you are a consummate team player who will thrive in a technology forward, rapidly growing and changing environment, quickly adapting to change and contributing your ideas to help our team continuously improve our operations and quality of care.

    POSITION OBJECTIVES: 

    The Nurse Care Coordinator will:

    • navigate complex health systems to remove barriers patients face when accessing healthcare;
    • serve as a bridge between, and resource for Providers and Care Coordinators; leverage a biopsychosocial framework to meet patients where they are, empower and motivate them to engage in recommended care, communicate with patients to understand their care needs and provide education to help improve outcomes.

     

    RESPONSIBILITIES: 

    • Complete mandated reports of communicable diseases, animal bites and suspected abuse or neglect to local departments of health and protective agencies; document accordingly in the electronic medical record.
    • Lead the bulk order processes for indicated screenings and labs. 
    • Provide follow up outreach to patients with normal values and escalate abnormal results to a member of the Provider team, document accordingly in the electronic medical record.
    • Coordinate the ordering and delivery of clinical and community services, including durable medical equipment (DME), document accordingly in the electronic medical record.
    • Liaise with pharmacies for medication verification and regarding issues with prescribed medications; work with the pharmacy and Provider team to ensure resolution, document accordingly in the electronic medical record.
    • Arrange medication deliveries. 
    • Secure prior authorizations required by insurance companies to help facilitate referrals ordered by the Providers to facilitate patient care.
    • Drive expansion of mobile health visits (nurse triage). 
    • Monitor daily patient flow, escalate potential delays in care and resolve swiftly.
    • Perform patient post-visit follow up calls and document accordingly.
    • Perform frequent outreach to monitor patient progress to ensure adherence and success, document outcomes of outreach in the electronic medical record.
    • Review medical records to identify patients who would benefit from additional care, perform outreach and arrange care accordingly.
    • Identify additional resources and interventions that will help patients successfully improve their condition.
    • Serve on care delivery committees as assigned.
    • Ensure a first class, concierge experience for all patients.
    • Collaborate with the UCM internal team and external vendors.
    • Other duties as assigned by management team
    • Adhere to all corporate Information Security policies and procedures.
    • Identify any breaches or potential breaches of corporate Information Security standards and communicate those to appropriate individuals.
    • Maintain integrity, availability, and confidentiality of all PHI.
    • Adhere to HIPAA standards for information security, privacy, and confidentiality

    EDUCATION:  Associates degree in Nursing required, Bachelor's degree preferred

    LICENSURE:  LPN required or Registered Nurse

    EXPERIENCE:

    • Medical- 2 years of medical practice (emergency/urgent/PCP setting, preferred)

    • Experience in one or more of the following areas of Nursing, preferred:

      • emergency/urgent care
      • primary care and/or mental health
      • Clinical case Management, Care coordination; insurance/managed care
