We Are looking for LPN's Full time and Part- Time for one of our Nursing & Rehab Centers. Responsibilities include but are not limited to:

Responsibilities:

Performs other duties as assigned;

Supervise nursing partners as assigned. Complete performance evaluations as required

Working with the interdisciplinary care team, to assure accurate patient assessment and development/revision of individualized plans of care;

Assures the personal dignity and physical safety of each patient. Assist patients to attain the highest level of self-care possible;

Assure that nursing partners have been assigned duties;

Relieve other shifts if needed

Maintains open and ongoing communication with patients and families, providing opportunity and encouragement to participate in decision making;

Leads by example and performs hands on care

Assume responsibility for assisting with meds and treatments as needed

Requirements:

dedicated to working with older adults;

Strong work ethic;

Punctual;

excellent standards of care;

experience as an LPN in an Assisted Living Facility preferred;

Team player;

Compassionate;

current LPN nursing license;

a great attitude required.
