    JOB SUMMARY

    Under limited supervision, responsible for the development and implementation of the UCHC outreach program. The outreach director conducts outreach, health education, and limited case management services to patients (and their families), supervise CAA/CEC and other outreach staff, effectively manage clinic resources, and positively promote the outreach program through appropriate community outlets. This position is also responsible for assessing patient and community needs and evaluating the impact of services and assisting with outreach activities within the community.

     

    ESSENTIAL FUNCTIONS

    1. Provide effective leadership, coordination and training of health resource staff and organization in aspects of health resource advocacy.
    2. Develop and maintain the outreach program infrastructure (plans, policies, procedures).
    3. Responsible for the distribution and monitoring of workflow and coordination of assignments for staff.
    4. Compile data for reporting as required by program and grant-related deliverables and prepare standard forms.
    5. Coordinate and execute health screenings and other related outreach activities.
    6. Establish positive collaborative relationships with appropriate community agencies.
    7. Perform direct outreach and education to target populations as listed on program grant and/or as directed by Unicare' s leadership.
    8. Provide flyers, handouts, and other forms of educational/informational materials to facilitate the Medi-Cal application process.
    9. Be familiar with and utilize the health access resource directory.

     

    QUALIFICATIONS

    • Bachelor’s degree or higher.
    • Minimum of two (2) years’ experience within community service, health, or social service sector, or other relevant work. Supervisory experience preferred.
    • Excellent organizational skills and analytical skills, and ability to multi-task.
    • Understanding of California health care programs desirable.
    • Excellent communication (oral and written), interpersonal skills and computer skills, specifically Microsoft Professional Office software programs.
    • Ability to use computer or other electronic device to utilize EHR system and companion software.
    • Fluency in English is required.
