    Job Description:

    We are currently looking for a SOCIAL WORKER to join our team. This position is a Full-Time Days position and is eligible for the Full-Time Benefits Package! 

    The SOCIAL WORKER (SW) provides assessment, planning, problem resolution and casework services to patients and their families according to age specific needs.  The SW collaborates with the referring physician when needed and completes all patient care documentation concisely and timely.  The SW will assist with the ongoing discharge planning needs of the patient.  The SW is available for patient/family referrals upon the request of the physician and/or patient.  The SW is to provide in-hospital consultations within 24 hours of notification by the hospital.  The SW is available by phone for consultation as requested by NCSC hospital staff, physicians or patient/family.

    Required Skills

    • Masters or Bachelor degree of an accredited program of Social Work.
    • Five years hospital experience is required. Experience with all age groups preferred.
    • Licensed Medical Social Worker in the State of Texas.
    • Possess the ability to deal tactfully with staff, physicians, patients, families and visitors.
    • Knowledge of Joint Commission, CMS, TDH standards, community resources, and third party payor sources.
