    PURPOSE:

    This position is a key role in the WSFC Continuum of Care’s Community Intake Center (CIC). The CIC was developed on the premise of having no wrong door to accessing critical supportive housing services.  The CIC Family Outreach and Housing Navigation Specialist will play a critical role providing access to assessment and basic mainstream services for people who are unsheltered or for whom the traditional front doors, the shelter system, are inaccessible to them.

    DUTIES AND RESPONSIBILITIES:

     

    ▪    Assist with intake and outreach efforts of the Continuum of Care to ensure all eligible people, with a primary focus on families with minor children are identified and assessed for prioritization with CIC.

    ▪    For identified families that are not defined as literally homeless, engage family in conversations and connections to other community resources within the community that will divert or prevent the family from becoming homeless.

    ▪   Maintain case files for Assessment Team.

    ▪   Assist with the screening and assessment of calls from the CIC line.

    ▪    Assist heads of household that do not otherwise have the support of a case manager with accessing their id, birth certificate, social security card and essential public benefits until they are assigned a housing case manager or have entered permanent housing.

    ▪   Participate in CoC’s improvement strategies working to decrease the length of time it

    takes for people experiencing homelessness to get housed.

    ▪    Manage HMIS data related to the SIEMER grant and upload data to SIEMER portal at least monthly.

    ▪    Assist in the management of HMIS data to ensure data is current, complete, and accurate to facilitate CIC prioritization planning.

    ▪   Other duties as assigned.

    ▪    ORGANIZES AND MAINTAINS LARGE QUANTITIES OF DATA: Understands that the housing programs and services provided by the Continuum of Care require large amounts of data to track to measure the effectiveness of the services and related programs.

    ▪   UNDERSTANDING OF PRINCIPLES OF DIVERSITY, EQUITY, AND INCLUSION.

    ▪    HOUSING FIRST: understands the primary principles of housing first, that no preconditions be made prior to assisting people who are homeless find and secure housing that are unrelated to meeting the terms of a standard lease.

    ▪    GIVES ATTENTION TO DETAIL:  Pays close attention to detail in performing data entry and in obtaining appropriate documentation for case files.

      ORGANIZES AND MAINTAINS LARGE NUMBER OF CASE FILES: Works well with organizing documents in case files in a specific format.                             

    ▪    COMMUNICATES EFFECTIVELY: Speaks clearly and expresses self well in groups and one- on-one conversations. Communicates well in presentations relating to data and information.

    ▪   OPEN AND WILLING TO WORK WITH PROGRAM PARTICIPANTS WITH SIGNIFICANT CHALLENGES:

    including substance use and mental health issues.

     

    JOB REQUIREMENTS:

    ▪   EDUCATION: BA preferred.

    ▪   WORK EXPERIENCE: Five years of experience or a college degree.

    ▪    SKILLS:  Data entry experience and working knowledge of Microsoft Excel, experience with HMIS and/or Service Point preferred.

    ▪   PEOPLE WHO HAVE EXPERIENCED  HOMELESSNESS ENCOURAGED TO APPLY.

       PHYSICAL REQUIREMENTS

       The position requires occasional movement throughout the office to access files, or papers 

        weighing up to 20 lbs.
