The Senior Community Service Employment Program offers seniors the opportunity to demonstrate their work ethic, enthusiasm, loyalty and life experience empowering them to improve their economic self-sufficiency. - Completes all required paperwork and data collection requirements including: completing/recording participant enrollment and ongoing paperwork and eligibility documentation.
- Keeps detailed case notes on all participants per SCSEP SOP and case note policy.
- Completes required exit and follow-up documentation.
- Delivers employment customer satisfaction surveys.
- Develops a minimum of two new employer relationships monthly to provide job opportunities for participants.
- Adheres to weekly and monthly program deadlines.
- Ensure that follow-up and retention is maintained per SCSEP performance requirements.
- Ensures data entry and documentation is compliant with payer source regulations and ESGW policy.
- Complies with federal and state practice requirements, HIPAA guidelines, CARF (if applicable), and ESGW requirements.
- Follows regular checkpoints and performance indicators to consistently meet quality standards and timelines.
- Maintains timely, complete and accurate record keeping.
- Utilizes quality and compliance tools to ensure requirements are met.
- Internal auditing of participant files
- QA review of case work entered into Sparq and Salesforce
- Monitor staff performance goals monthly
- QA retention metrics

Required Experience
- 0-2 years
- Proven experience with job development and/or employment placement.
- Proven time management skills.
- Experience working with persons with diverse backgrounds
- High School/Equivalent
