Job Summary
We are seeking a Unlicensed Intake Coordinator to join our team. In this role, you will collect, manage, and report on the referral and scheduling information. Assists in the coordination of patient care under the direction of the Office Manager and Direction of Nursing. The ideal candidate is empathetic, highly organized, and possesses excellent customer service skills.
Responsibilities
Answers phones, receives all intake information for paraprofessional cases and enters the appropriate information in the computer
Completed intake forms and routes them appropriately for admission approval
Assists in the scheduling of appropriately skilled person to fill the case, inputs scheduling information in the computer and continues to assist in the coordination of the scheduling
Schedules replacement workers as needed and inputs scheduling information
Assists in communication with patients/families on home health aide related issues
Takes telephone referral information if no nursing staff is available and pass referral as soon as possible
Communicates on a routine basis with contract agencies to maintain smooth relations and to obtain needed information
Documents and reports changes in patient status, in service requirements, in and employee staff assignment and in particular, any unsatisfactory performance
Performs other duties as directed
Qualifications
High school diploma or GED
Previous experience as a Home Care setting preferred but not required
Highly organized with attention to detail
Must be computer literate
Comfortable with Microsoft Office and other computer programs
Ability to multitask and prioritize projects
Excellent customer service and interpersonal skills
