    Job Description

    OUTREACH MANAGER

    The Outreach Manager (OM) plays a key role in conducting outreach to healthcare providers and social service organizations and in managing the Outreach Specialists. Referrals from healthcare providers and social service organizations are critical to building the volume of enrolled patients for Upward Health. These referrals may come from a variety of organizations, including primary care practices, food assistance agencies, or hospitals. The OM is responsible for building partnership and trust in the community with these providers and organizations. The OM will oversee the hiring, training, and day-to-day management of the Outreach Specialists. The OM will assign caseloads to the Outreach Specialist to work on a weekly and monthly basis and will triage new referrals that are received. Ongoing guidance and support for the Outreach Specialist will be a daily need.

    The OM should be flexible as the responsibilities of the role will fluctuate with business demands. For example, the OM may work with providers who contract with Upward Health to deliver Facilitated Virtual Care visits or even fill in for an Outreach Specialist directly. Motivation to build our enrollment, initiative to expand our community footprint, and flexibility to manage growth and change will make for a successful individual in this role.

    KEY RESPONSIBILITIES:

    • Educate healthcare providers and social services organizations about the value that Upward Health can provide them and their patients
    • Build relationships to encourage referrals from healthcare providers and social service organizations; manage all inquiries from these organizations
    • Design and execute processes to facilitate seamless referrals from organizations
    • Triage inbound referrals and follow-up directly or assign to Outreach Specialist for follow-up to ensure quick response time
    • Lead Outreach Specialists to execute successful outreach approach and complete enrollments
    • Establish outreach and enrollment goals, manage processes to achieve goals, and establish measurements to assess progress
    • Monitor workloads of Outreach Specialists and executes adjustments in assignments to ensure goal achievement
    • Lead hiring of new Outreach Specialists and training of both new hires and on an ongoing basis for improvements in messaging and strategy
    • Identify trends in enrollment activities and modify approach accordingly
    • Develop and lead Outreach reporting included key performance indicators that are shared weekly with management
    • Collaborate with Clinical Services team for patient handoffs and reverse handoffs
    • Share information about healthcare providers and social service organizations with Clinical Services team that may be relevant in their work with enrolled patients
    • Represent Upward Health at community events

    OUR IDEAL CANDIDATE:

    • Ability to clearly communicate Upward Health’s value proposition
    • Experience and comfort with public speaking and delivering presentations
    • Ability to quickly establish trust and build strong relationships with patients, providers, and social service organizations
    • Active listening skills, quick thinking, and able to both be compassionate and respond to objections
    • Versatile ability and cultural competency to interact with the right content and tone with administrators, clinicians, and patients
    • 3-5 years’ experience managing a diverse group of individuals with different backgrounds and education levels
    • Demonstrated ability to successfully lead a team, achieving high performance by the individuals on the team
    • Skilled using data and reporting to inform own and team’s work
    • Skilled using computer for documentation, communication, and organizing work
    • Must have reliable transportation
    • Must be fluent in English, bilingual a plus
    • Able to work flexible hours, including occasional night/weekend work
    • 3-5 years outreach experience to healthcare providers and social service organizations
    • Bachelor’s degree preferred. Consideration of relevant experience in lieu of educational requirement would include 5-7 years of outreach, management, or other healthcare-related roles.
