What You Need to Bring:

• A year or more of Medical Assistant experience.
• Minimum 6 months in Internal Medicine or Family Medicine.
• Skilled in back office Medical Assistant tasks.
• Medical Assistant Diploma or Board Certification.
• Active driver's license and reliable vehicle.
• Ready for an average of 6 patient home visits per day.
• Proficient with EMR Systems and tech-savvy.
• Available 8am-5pm Monday - Friday.

Extra Skills We Love:

• Experience with Annual Wellness Visits.
• Home visit experience.
• Geriatric care experience.
• Fluent in multiple languages, including Spanish, Korean, Vietnamese, and Mandarin.

Your Role:

You'll be at the forefront, visiting an average of 6 homes daily for Annual Wellness Visits. Here's what you'll do:

• Verify Primary Care Provider details.
• Conduct Medication Reconciliation for accurate medication lists.
• Confirm patient allergies.
• Administer the Annual Wellness Visit (AWV) Questionnaire.
• Record vital signs.
• Perform up to 6 Preventive Screenings (PADnet, DPNCheck, Dry Blood Spot Fingerprick test, FIT Kit, Urine Microalbumin Dipstick, DRE exam). Past experience performing this screenings is strongly preferred.

Compensation & Benefits:
