    RESPONSIBILITES:

    Provide care as directed by Clinical Manager and according to the client's personalized care plan

    Duties may include but not limited to: bathing, dressing, grooming, toileting, mobility/transfers, med reminders, meal prep.

    Assist with light housekeeping, linen changes, laundry, food shopping/errands. Accompany client to medical appointments if needed

    Communicate effectively with clients, families and clinical management/office staff

    Ability to recognize and report change in client condition

    Maintain trusting relationships with the clients and family by consistently demonstrating kindness, compassion and respect

    REQUIREMENTS:

    Active CNA state license or proof of completion of a valid home health aide training program (required).

    High school diploma or equivalent.

    1 year of health care experience; home care experience working with adults preferred.

    Current CPR certification

    Must have reliable automobile, valid driver's license and auto insurance.
