    SUMMARY: The Transportation Van Driver will be responsible for performing all duties related to scheduling, communicating, and transporting the center''s residents in the center van under the direction of the Administrator. In addition to facilitating the center operations and care to our patients/residents requiring long term and/or rehabilitation care. The Transportation Van Driver will also, support the company''s mission, vision, core values, and provide excellent customer service to associates, residents, visitors, and guests. ESSENTIAL DUTIES AND RESPONSIBILITIES:

    • Transport residents to and from appointments.
    • Schedule appointments for residents.
    • Monitor appointment book daily.
    • Manage the appointment schedule (i.e. if several appointments are on one day, try to re-schedule and space them out, get with Unit Managers prior); if other methods of transportation are needed (i.e. Medivan, AMR, or Medicaid transport) schedule accordingly.
    • If assistance is needed with a transport, communicate the need to supervisor and scheduler the week prior to appointment.
    • Assigning someone else to monitor appointment book on days off; reporting errors in system to DON. Following transportation protocol.
    • Complete transportation communication forms: Notify resident and family of appointment. File communication forms under social service tab by the end of each week.
    • Complete Van Securement Checklist prior to each transport.
    • Report to Maintenance Director any vehicle maintenance issues.
    • Clean van daily after trips.
    • Transfer residents to and from appointment visits by wheelchair.

    Required Skills

    PHYSICAL DEMANDS AND ENVIRONMENT:

    • Ability to drive large passenger vehicles.
    • Perform general van inspection prior to each use.
    • Frequent sitting, standing, bending, lifting, grasping, pushing, pulling, and moving intermittently during working hours.
    • Subject to frequent interruptions and distractions.
    • Willing to work beyond normal working hours and during disastrous circumstances, as needed.
    • Willing to work in other positions temporarily, as needed.
    • Attend and participate in continuing educational programs.
    • Must be capable of performing the essential job functions of this position with or without reasonable accommodation.
    • Must be able to relate positively and favorably to residents, families, and outside agencies and to work cooperatively with other associates.
    • May be subject to infectious diseases, substances, contaminated articles, cuts, bruises, falls, odors, etc.
    • Must be able to function independently, have personal integrity, have flexibility, and the ability to work effectively with or without other personnel and co-workers.
    • Ability to remain calm and cope with the mental and emotional stress of this position. OTHER REQUIREMENTS:
    • Understand, comply with, and promote rules regarding residents'' rights.
    • Maintain an orderly, confidential, and safe work environment; in addition, to HIPAA guidelines.
    • Practice universal precautions at all times.
    • Adhere to all company and departmental policies and procedures.
    • Perform other duties as assigned.

    Required Experience

    QUALIFICATIONS:

    • Must be at least 18 years of age.
    • Completion of high school education or otherwise show ability to read, write, and follow oral and written directions.
    • Basic computer and office equipment knowledge.
    • Must be capable of maintaining regular attendance, as required.
    • Must be able to work weekend and overtime as required.
    • Must have strong organizational skills.
    • Must be able to follow and/or provide strong communication skills effectively through written and oral delivery in a manner appropriate for a healthcare/office setting.
    • Must be able to problem solve, if needed.
    • Must meet all local health regulations and pass post-employment exam, if required.
