    Job Summary:

    The Visit Nurse Practitioner provides comprehensive healthcare services to patients in their homes or community settings. Responsibilities include conducting physical exams, diagnosing and managing acute and chronic conditions, prescribing medications, and developing care plans in coordination with the interdisciplinary team. This role focuses on delivering high-quality, patient-centered care that supports patient autonomy and reduces hospital admissions.

    Key Responsibilities:

    Perform in-home patient assessments, including history taking, physical exams, and diagnostic evaluations.

    Diagnose and manage acute and chronic medical conditions.

    Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.

    Prescribe medications and order diagnostic tests and treatments as appropriate.

    Monitor patient progress and adjust treatment plans as necessary.

    Educate patients and caregivers on disease management, medication use, and health promotion.

    Coordinate care with other providers, home health staff, and community resources.

    Document all visits and patient interactions accurately and timely using electronic health records (EHR).

    Participate in quality improvement initiatives and comply with regulatory standards.

    Provide end-of-life care and symptom management when working in hospice settings.

    Qualifications:

    Master’s or Doctorate degree in Nursing (MSN/DNP)

    Current and valid Nurse Practitioner license in the state of practice

    Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology NP (AGNP), or appropriate specialty

    DEA license (if prescribing controlled substances)

    Minimum 1–2 years of NP experience, preferably in home health, geriatrics, or primary care

    Current BLS/CPR certification

    Valid driver’s license and reliable transportation

    Skills and Competencies:

    Strong clinical assessment and diagnostic skills

    Ability to work independently and manage a flexible schedule

    Excellent communication and interpersonal skills

    Proficiency in EHR systems and mobile health documentation

    Compassionate, patient-centered care approach

    Knowledge of regulatory standards (e.g., Medicare, HIPAA)
