VitalCheck Wellness is seeking a committed and compassionate Patient Care Coordinator with experience in phlebotomy to perform skilled specimen collections and vital signs for employees at private businesses in and around Ithaca, NY and Penn Yan, NY . We are looking for someone with excellent interpersonal skills and ability to create a supportive environment.

Will consider an LPN, Phlebotomist, or a CNA/ MA and Paramedic with phlebotomy experience (Phlebotomy certificate preferred).

_** ** Responsibilities:**

• Visit patient at their residence, take vital signs, perform vision test and collect specimens accurately (Phlebotomy & urine drug test)
• Record patient information, including insurance information, and utilize electronic health records.
• Discuss and consult health history, diagnosis, and needs with patients.
• Establish and maintain relationships with service providers.
• Maintain professionalism and treat patients with respect.
• Provide excellent customer service to patients.
• Collect data and create spreadsheets.

Requirements:

• Must have experience in vital signs.

• Must have phlebotomy experience. Certified phlebotomist preferred. Urine drug test experience and certification strongly preferred.

• Valid license/certification.

• Customer service-oriented and have great bedside manner.

• Strong communication skills; both written and verbal.

• Comfortable working independently or with little to no supervision.

• Must be up to date with COVID-19 vaccine.

• Must be able to reliably commute to patients' residency around Ithaca, NY and Penn Yan, NY .
