    Overview: As a Home Health Registered Nurse, you will play a critical role in providing high-quality, compassionate care to patients in the comfort of their own homes. Your responsibilities will include assessing patient needs, developing care plans, administering treatments, and educating both patients and their families on managing health conditions and promoting wellness. This position requires strong clinical skills, excellent communication abilities, and a genuine commitment to improving the lives of others.

    Key Responsibilities:

    Conduct comprehensive assessments of patients' health status and individual needs.

    Develop personalized care plans in collaboration with patients, their families, and other healthcare professionals.

    Provide skilled nursing care, including wound care, medication management, and monitoring vital signs.

    Administer treatments and medical procedures as prescribed by physicians.

    Educate patients and caregivers on disease management, medication administration, and lifestyle modifications.

    Coordinate with other members of the healthcare team to ensure continuity of care and effective communication.

    Document patient care activities accurately and timely in electronic health records.

    Monitor and evaluate patients' progress, adjusting care plans as needed to achieve optimal outcomes.

    Provide emotional support and advocacy for patients and their families.

    Adhere to all regulatory guidelines, including Medicare and Medicaid regulations, as well as agency policies and procedures.

    Qualifications:

    Current Registered Nurse (RN) license in the state of NJ.

    Bachelor's degree in nursing (BSN) preferred.

    Minimum of two years of clinical experience, preferably in home health or a related setting.

    Strong clinical assessment and critical thinking skills.

    Excellent communication and interpersonal abilities.

    Ability to work independently and as part of a multidisciplinary team.

    Proficiency in using electronic health records and other healthcare technology.

    Compassionate and patient-centered approach to care.

    Reliable transportation and valid driver's license.

    CPR certification required.

    Physical Requirements:

    Ability to lift and move patients safely, as needed.

    Comfortable working in various home environments, including stairs and uneven surfaces.

    Ability to withstand long periods of standing, bending, and walking.

    Good manual dexterity and hand-eye coordination for performing nursing procedures.
