    DHS HOUSING (ODR)

    This Office of Diversion and Reentry Program connects chronically homeless, dual diagnosed, frequently incarcerated individuals with a permanent housing solution.  Embraces a Housing Readiness Process, that evaluates each individual for permanent housing placement.  Multi agency and inter-disciplinary provides wrap-around support.  Connecting the most vulnerable to housing.  Housing placement employment services, mental health services, benefit services, case management, drug and alcohol treatment.

    JOB SUMMARY AND PURPOSE

    The Program Coordinator I provides experience-based competencies and administrative assistance to further the Grant missions for VOALA: outreach, diversion, intake, assessment, trauma-informed case management, job development, SUD and housing assistance.  The Program Coordinator I supervises the provision to clients of individualized care through access to service linkages, counseling, treatment planning, and various crisis intervention methods in order to prepare individuals for further success.  

    DUTIES AND RESPONSIBILITIES 

    • Achieve grant objectives by supervising the program staff, providing expertise, and monitoring service delivery, while providing information to the Director, highlighting progress in reaching goals;
    • Coordinate day-to-day operations of program service providers, overseeing intake, creation of service plans and delivery of services to clients;
    • Conducts weekly case conferencing and monthly case files audits with staff to assist participant progress, and assure records are complete and accurate;
    • Monitor program compliance to VOALA policies and required procedures; maintains documentation of all program procedures;
    • Assist Director with implementing policies, procedures and forms that facilitate the effective operation of intensive case management and delivery of high-quality services – train and mentor staff
    • Steward of program assets and budget, oversees asset protection measures
    • Coordinate outreach and collaboration efforts with community partners and stakeholders
    • Other duties as assigned

    Qualifications

    REQUIREMENTS:

    • Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications

    EDUCATION:

    • Bachelor's degree in Social Work (or related discipline) or a combination of relevant education and experience (4 years).

    EXPERIENCE:

    • 1 year of supervisory experience in a social services organization (leading programs, projects, or staff)

    KNOWLEDGE:

    • Strong knowledge of case management ethics, HIPAA, Programmatic standards of operation and clinical engagement, clinical documentation methods, client focused ideology.

    TECHNOLOGY SKILLS:

    • Strong knowledge of case management ethics, HIPAA, Programmatic standards of operation and clinical engagement, clinical documentation methods, client focused ideology.

    SKILLS AND ABILITIES:

    • Demonstrated strong written and verbal communication,
    • Deep understanding, ability to assess, and consult with team on participant acuity and appropriate levels of care through treatment team meetings.
    • Ability to coach and mentor staff

    INTERPERSONAL CHARACTERISTICS:

    • Client- focused, Prompt, collaborative/team player, ability to delegate, mentorship skills, effective stress management, leadership skills, conflict resolution/mediation.

    PREFERRED QUALIFICATIONS:

    • Master of Social Work
    • Ability to work with people from various backgrounds and/or limited English capabilities
    • Knowledge about community resources
    • Experience connecting clients to resources
