Description

Adams Case Management is seeking a Waiver Case Manager to support individuals receiving waiver services in Ramsey County.

The Case Manager works with individuals, families, and providers to coordinate services and ensure individuals receive the supports needed to live safely and independently in the community.

Responsibilities

Provide case management for individuals receiving CADI, BI, CAC, and DD waivers

Conduct face-to-face visits

Develop and maintain CSSP

Coordinate services with providers and county staff

Maintain documentation in SSIS

Qualifications

Required:

Bachelor’s degree in Social Work, Psychology, Human Services, or related field

Valid driver’s license

Preferred:

Experience with waiver services or case management

Experience with SSIS

Waiver case management experience is preferred but not required. Training will be provided.
