    • Title: Social Determinants of Health (SDOH) Advocate
    • Pay: 19.70-24.62 based on experience
    • Schedule: Full-time; Monday – Friday; with the option for 4-10’s or 5-8’s. Will have the option for some hybrid work.

    Additional qualifications include:

    • Bachelor's degree in public health, social work, or related field preferred
    • Experience working with a multicultural, varied socioeconomic client population in primary care, community health, or similar setting
    • Knowledge of community-based organizations and resources
    • Excellent oral and written communication skills
    • Smart time management and organizational abilities

    Bonus Points:

    • Fluent in Spanish (¡Hablas español? ¡Perfecto!)
    • Experience working with diverse and underserved populations
    • Familiarity with community resources and social service agencies in the local area
    • Experience in grant writing or fundraising for community programs
    • Knowledge of health equity principles and strategies
    • Certification in community health or related field
    • Experience with data analysis and reporting for community health initiatives
    • Demonstrated commitment to social justice and advocacy efforts

    Your Role: As a Social Determinants of Health Advocate, you'll play a vital role in addressing the social determinants of health for our patient population. Your duties will include:

    Community Engagement:

    • Developing and maintaining relationships with community-based organizations to build a network of resources addressing social determinants of health
    • Managing project-based community engagement strategy, development, and execution
    • Creating written content for materials for community engagement and patient involvement, working closely with Wallace's Marketing team
    • Implementing strategies to engage all communities, especially underserved and marginalized people, impacted by transportation and housing projects

    Team Collaboration:

    • Working collaboratively with clinical staff, Equity team, and Media and Public Affairs team to ensure streamlined messaging and responsiveness
    • Attending and staffing community events and briefings as needed
    • Collaborating with community-based organizations and UniteUs to improve platforms as they relate to our patient population
