    Key Details:

    • Title: Diabetes Educator – RN
    • Pay: $42 - 50 per hour based on experience
    • Schedule: Full-time; Monday – Friday; variable shifts between 8 am – 6:30 pm with the option of 5-8’s or 4-10’s and will require some evenings for group classes
    • Location: 18633 SE Stark St., Suite 401 – Portland, OR 97233

    Who You Are: You are a compassionate RN with exceptional communication skills and a dedication to improving outcomes for diabetic patients. You thrive in collaborative environments and excel in patient education, case management, and program coordination.

    Basic Qualifications:

    • Minimum of one (1) year working in population health in an outpatient clinic providing nursing care including chronic disease management, diabetes care, and patient education
    • Associate's degree or higher in Nursing or directly related field
    • Unencumbered Registered Nurse License in Oregon
    • Diabetes Educator Certificate within 6 months of hire

    Additional Requirements:

    • Experience in diabetes teaching, nursing advice, and insulin instruction
    • Previous experiences in case management, medication management, and program coordination
    • Must be CDE certified (or become certified within 6 months of hire) and maintain certification throughout employment
    • Current and thorough understanding of Diabetes treatment management and education including dietary modifications, exercise, and medication regimens
    • Knowledge of a variety of glucometers
    • Must be self-directed, able to work independently with little supervision while meeting performance metrics
    • Detail-oriented
    • Excellent organizational and time management skills
    • Strong communication and telephonic skills
    • Bilingual in Spanish

    Preferred Qualifications:

    • Knowledge of EPIC system
    • Bachelor's degree in Nursing

    Your Role: As an RN Diabetes Educator, you'll play a vital role in improving outcomes for our diabetic population. Your duties will include:

    Diabetes Education and Case Management:

    • Designing, implementing, and maintaining clinical and educational programs/activities for diabetic patients
    • Utilizing Motivational Interviewing or other behavior change techniques to coach and assist patients with self-management
    • Providing direct care to patients and families including Case Management of chronically ill patients with varying social, economic, emotional, developmental, and medical issues
    • Assessing, planning, implementing, and evaluating the effectiveness of individual patient care plans based on the nursing process
    • Developing and implementing patient education through the use of available tools such as MyChart or CareMessage in addition to small group classes and one-to-one appointments
    • Managing the list of patients with A1cs above 9 – providing care management, appointments, medication reconciliation, consistency in care, follow-up, and chronic disease management

    Team Collaboration:

    • Working closely with the Clinical Quality Manager and the Medical Director to review metrics associated with diabetes as we pursue continuous quality improvement
    • Reducing care gaps by encouraging or assisting with preventive care and chronic care management
