    The Crisis Respite Program (CRP) Case Manager is a member of a multi-disciplinary team responsible for providing ongoing assessment, crisis stabilization, engagement and short-term case management services to homeless and mentally ill adults. This position will work closely with other CRP clinicians, HOST project staff and Shelter staff. Services will be provided at DESC facilities and in the community.

    MAJOR DUTIES AND RESPONSIBILITIES



    • As a member of a multi-disciplinary team, provide multi-axial assessments, crisis intervention, stabilization and short-term engagement to homeless adults referred to CRP.

    • Receive and screen referrals from approved referral sources and coordinate transfer to CRP.

    • Assume primary responsibility for coordinating all aspects of clients'support and treatment plans while in the CRP. This includes: evaluating the psychiatric, substance abuse, and housing needs of clients, and advocating for CRP clients to access the most effective disposition available in the community.

    • With clients, create disposition plans to community resources and provide needed assistance and advocacy to assure best possible connection is made.

    • Develop expertise and strong working collaborative relationships with the Harborview Psychiatric Emergency Services staff and the Mental Health Liaison, as well as with ---back door--- providers of mental health treatment, substance abuse treatment and housing services.

    • Provide on-call coverage for CRP clients as needed.

    • Consult and collaborate with other DESC staff to ensure continuity of care for clients in the CRP.

    • Develop and maintain cooperative relationships with other DESC programs providing services for homeless and mentally ill persons.

    • Participate in psychiatric consultation, supervision, program meetings and in-service trainings; participate in clinical reviews and case conferences for clients.

    • Comply with the agency's clinical accountability policies and procedures; maintain current and complete clinical records; participate in quality assurance reviews when assigned.

    • Maintain clean and healthy environment in CRP.

    • Coordinate work closely with other CRP staff and provide guidance as needed to CRP Counselors.

    • Enforce Shelter rules and consult with shelter staff regarding the consistent application of shelter rules.

    • Be respectful of clients and staff.

    • Other duties as assigned.


      MINIMUM QUALIFICATIONS



      • For Case Manager I: Master---s degree in social work, psychology or other behavioral science OR BA/BS degree in a social or behavioral science or relevant field plus two year---s relevant work experience.

      • For Case Manager II: Masters degree in a social or behavioral science or relevant field AND two years post-graduate experience with chronically mentally ill adults who have similar backgrounds as DESC clients. Equivalent relevant experience may not be substituted for degree requirement.

      • Ability to meet state requirements for registration as a counselor.

      • Interest in and experience in working with homeless, mentally ill, and/or substance abusing individuals.

      • Knowledge of, and ability to assess, mental and substance abuse disorders.

      • Experience or interest in working with clients who are difficult to engage and refer to traditional programs.

      • Ability to remain open and receptive to constructive feedback.

      • Bi-cultural or bilingual background preferred.

      • Ability to work effectively with clients displaying a wide range of unpleasant and/or bizarre behavior.

      • Ability to communicate and work effectively with staff from various backgrounds.

      • Subscribe to the philosophy of cooperation and continuity across programs and of consideration and respect for clients and staff.

        The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee will be required to sit, stand, walk and go up and down stairs. The employee will be required to bend, stoop, kneel, and reach as well as lifting and/or carrying items weighing up to 40 pounds. The ability to operate computer hardware systems is also required. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Considerable stress may occur.
