    CLINICAL QUALITY PROJECT COORDINATOR

    DEPARTMENT: Quality/Medical

    REPORTS TO: Clinical Quality Manager

    STATUS: TEMPORARY PART-TIME (20 HRS/WK)

    FLSA: Non-Exempt

    POSITION SUMMARY

    The Project Coordinator will assist with the facilitation of WellSpace Health’s Quality Plan. Responsible for tobacco cessation screening and enrolling patients into activities related to the implementation of an evidence-based, quality improvement project to increase the effectiveness of tobacco cessation efforts including facilitated referrals to Kick It California (KIC). Will optimize the use of clinical data systems to enable ready assessment of population health metrics including: smoking prevalence in the clinic patient population, referral rates, counseling utilization rates, and quit rates. Provide tobacco-using patients with evidence-based guidance related to diet, physical activity, and stress management; and adopt and implement recommended tobacco-free clinic policies.

    REQUIRED QUALIFICATIONS

     

    • Bachelor's degree in public health, social work, education or related field preferred OR a combination of education and three years + (3) years of project coordination in lieu of degree.
    • Experience in program development in delivering human services; facilitating programs and policies related to behavioral health services preferred
    • Experience in community engagement/education; Tobacco cessation experience preferred
    • Fluency in additional languages preferred.
    • Demonstrate commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, and/or physiologically impaired.
    • Communicate clearly and concisely, both verbally and in writing and demonstrate a high level of listening skills.
    • Excellent interpersonal skills.
    • Strong customer service experience including active listening, problem solving, and the ability to remain calm in emotional or stressful situations.
    • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
    • Compliance with all state and federal laws and regulations as they pertain to position including infection control, sexual harassment, Mandated Reporting, Scope of Practice, OSHA, etc.
    • Adhere to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
    • Assist and support the Center/Department/Program to meet standards of High Reliability.
    • Must have valid driver’s license and insurable driving record.
    • Ability to work extended hours and/or weekends related to Health Center hours of operation or special events and programs.

    ESSENTIAL RESPONSIBILITIES

     

    • Primary day-to-day point of contact for CDPH/CTCP communication to the agency.
    • Responsible for overall and day-to-day management to implement and evaluate the project.
    • Provide oversight of the Work Plan, budget, progress reports, and invoices.
    • Oversee maintenance of required documents for auditing purposes.
    • Implement evaluation activities.
    • Prepare required reports, perform data entry.
    • Visit the HLCI website on a monthly basis for networking, and to access educational and policy materials for the project.
    • Submit periodic progress reports to CTCP via the Online Tobacco Information System (OTIS).
    • Work closely with Clinical Quality team.
    • Assist with related medical record data collection.
    • Assist with clinical quality projects and initiatives.
    • Maintain and adhere to confidentiality, and privileged communications (patient, employee, and organization).
    • Perform other duties as assigned.

     

    PHYSICAL DEMANDS AND WORK ENVIRONMENT

    The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

    The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
