Position Summary The Intervention Specialist provides intensive case management to youth aged 15-24 injured by community violence. Works in cooperation with SVIP team, Kaiser Permanente partners at the KP Trauma center, UC Davis Wraparound Team and other community partners. The Intervention Specialist will support victims of violence navigating the CalVCB process, co-facilitate psycho-education and support groups, may facilitate outreach presentations, and participates in collaborative meetings in the community. Required Qualifications * High School Diploma or GED required. * Bachelor's in Social Work, Public Health, Psychology or related field OR two (2) years of social services, case-management of experience in lieu of education * Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment. * Demonstrated commitment to working with youth. * Local community member who is knowledgeable of the dilemmas faced daily by youth in low-income areas. * Bilingual in Spanish preferred; or other languages desirable. * Knowledge of urban youth issues, specifically youth violence; evidence based intervention techniques. * Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired. * Must be able to present self and program in a professional manner; Highly organized and detail oriented; Punctual & extremely reliable; Ability to work in stressful situations. * Able to work a flexible schedule including some evenings and weekends. * Must have reliable insured car with valid California Driver's license, insurable driving record and DMV clearance. * Communicate clearly and concisely, both verbally and in writing. * Proven time management skills to complete case management services and administrative duties. * Ability to engage with and support youth and families to achieve client driven service plan. * Proven experience in navigating community resources and services. * Ability to follow ethical and legal requirements in serving youth. * Ability to effectively use Microsoft Excel, Word, Outlook email and calendar and Electronic Health Records systems. * Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing. * Assist and support the Center/Department/Program to meet standards of High Reliability. * Must have valid driver's license and insurable driving record. Essential Responsibilities * Provides case management services to a maximum caseload of 17 victims of community violence. * Supports youth in creating and achieving a one year goal plan. * Provides transportation to service recipients as needed. * Provides hospital bedside services. * Works closely as a team and independently in the field. * Provides home and community visits. * Follows policies and procedures of the organization and program partners. * Partners with SVIP Clinician to support youth in receiving mental health services. * Collaborates with SVIP Lead Intervention Specialists and team.
