Position Summary The Outreach Specialist supports program growth by making initial contact with potential patients, providing information about Enhanced Care Management services, and enrolling patients into a designated health plan. Enhanced Care Management provides care coordination services to the most vulnerable populations in the greater Sacramento Region. The goal of these services is to provide high quality, comprehensive, whole person care to improve health and wellness outcomes for our patients. The Outreach Specialist is responsible for keeping detailed records of their efforts and communicating regularly with Supervisors and Program Managers. Required Qualifications * High School diploma or equivalent. Bachelor's degree preferred. * Minimum six (6) months case management or outreach experience, and/or lived experience utilizing Medicaid or other social service systems. * Must be able to work collaboratively with others in a team environment, respecting and valuing the perspectives and experiences of others. * Familiar with Harm Reduction, Motivational Interviewing, Client Centered theory and Trauma Informed Care. * Able to maintain and understand appropriate roles, and boundaries among patients, peers, and WellSpace partners/stakeholders. * Able to function effectively in a fast paced environment. * Ability to write progress notes and maintain detailed, statistical records. * Demonstrated commitment to the provision of services for the underserved and sensitivity working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired. * Must demonstrate a high level of verbal, writing and listening skills. * Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality. * Current CPR and first aid certificate or willingness to be trained required within first (90) ninety days of employment. * Computer literacy including use of email, Microsoft Word, Excel, and EHR. * Adherence to infection control procedures, including but not limited to, standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing. * Assist and support the Center/Department/Program to meet standards of High Reliability. * Must be willing to visit clients using personal vehicle. * Must have a valid CDL and must be insurable. Essential Responsibilities * Reach out in-person, by phone or by mail, to explain ECM services and enroll patients. * Review Target Eligibility List daily, as well as other referral sources, to determine ECM program eligibility * Assure Patients are assigned to the appropriate health plan Supervisor, once enrolled * Keep detailed records of efforts, regardless of outcome, including spreadsheets and reports. * Document all interactions with and on behalf of the patient in the appropriate patient care systems, including the patient's Electronic Health Record (EHR). * Maintain consistent attendance and punctuality. * Attend regular meetings with management and team. * Perform other duties as assigned.
