GENERAL SUMMARY: Under general supervision, plans, organizes and conducts a program of activities therapy for residents in accordance with individual resident needs, facility goals and objectives and federal and state regulations.

DUTIES AND RESPONSIBILITIES: The following are essential job accountabilities: 1 Develops and monitors the activity program(s) in the Transitional Care Center. 2 Assesses resident recreational therapy needs (including leisure interests) within 48 hours of admission to the Transitional Care Center. 3 Establishes resident activity goals utilizing a collaborative process. 4 Maintains daily log of resident attendance at all activity functions. 5 Documents resident participation in activity programs in recreational therapy progress notes weekly and as indicated by change in level of residents condition. 6 Provides instruction and training to residents who have assessed need for adaptive equipment and material. 7 Facilitates resident orientation program ensuring that resident concerns are forwarded to the appropriate individuals for resolution. 8 Reports changes in the residents behavior and physical condition noted during activity participation to the charge nurse. 9 Provides a conducive atmosphere for resident activity pursuits including holiday themes. 10 Coordinates special activities and functions with community services. Required Skills

Bachelor's degree (graduate of an accredited program in therapeutic recreation)

Therapeutic Recreation certificate required
