    GENERAL SUMMARY:

    Under general supervision, provides physical therapy services at an entry to intermediate skills level to individual patients according to the principles and practices of physical therapy and works collaboratively with interdisciplinary treatment team. Duties and Responsibilities: The following are essential job accountabilities: 1 Evaluates patient using diagnostic and prognostic muscle, nerve, joint and functional ability tests. 2 Plans individualized physical therapy program for each patient including exercises, therapeutic modalities and procedures. 3 Directs and supervises patient in active, active assisted, passive exercises, muscle re-education, and functional training using appropriate equipment as needed. 4 Provides instructions in therapeutic procedures to be continued by the patient. 5 Coordinates physical therapy activities in collaboration with the other members of the interdisciplinary team to facilitate achievement of goals established. 6 Conducts or participates in training medical and nursing personnel in physical therapy techniques and objectives. 7 Utilizes safe patient handling techniques for all patient transfers, lifts and positioning. 8 Provides supervision of PTAs and Rehab Aides as required by PA practice act. DEPARTMENT DESCRIPTION:

    WellSpan VNA Home Care’s staff of nurses, aides, therapists and other caregivers works with physicians to provide the services necessary to treat patients in the comfort of their own home.

    WellSpan Rehabilitation therapists who work in home health have a diverse caseload, including orthopedic, neurologic, geriatric, lymphedema, chronic pain and postsurgical patients. As part of the team, you will help provide stimulating and challenging care to patients in the home environment.

    Our team takes a multidisciplinary approach to patient care, which requires you to work closely and coordinate care with other therapy disciplines and nursing. Caregivers within our integrated health system work hard to provide open communication and smooth handoffs among various levels of care.

     

    Required Skills

    Physical Therapist license

    Valid driver's license in good standing

    Required Experience

    2-3 years general PT experience

    at least 1 year home health experience preferred
