Duties
Provides advanced professional input to complex Case Management assignments/projects. Supports health promotion and disease prevention and care management services to the plan's members. Explains, educates, reinforces, and coaches members on their tailored self-help plans. Identifies problems, sets goals, and determines the individual's health status based on assessment. Establishes goals for each individual regarding educational and health care needs. Communicates with primary care and/or treating physician to request specific health information and/or to provide an update of an individual's health care status. Supports and provides direction to more junior professionals. Works autonomously, only requiring “expert” level technical support from others. Exercises judgment in the evaluation, selection, and adaptation of both standard and complex techniques and procedures. Utilizes in-depth professional knowledge and acumen to develop models and procedures, and monitor trends, within Wellness Coaching.

8-hour shifts between 7 and 4:30

Education
Requires B.S. or B.A in public health, exercise science or psychology. RD or LDN is also accepted
NWC certification preferred
3+ Years experience

End Date: 12/1/2025 (this coverage is for an FTE on short term disability, could go longer should the LOA last longer)
