    Job Description

    We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team in Streamwood, United States, on a PRN basis. As a Wellness Nurse, you will play a vital role in supporting the health and well-being of our residents through clinical monitoring, medication management, and comprehensive care coordination.

    • Conduct resident health assessments and screenings during admission process
    • Develop and maintain resident care plans according to individual needs
    • Administer medications and treatments following physician orders
    • Coordinate care services with external healthcare providers and resident physicians
    • Monitor and document resident health status, including significant changes or unusual observations
    • Ensure compliance with state regulations and facility policies
    • Support and guide caregivers and medication personnel in delivering quality care
    • Participate in quality assurance activities and maintain clinical excellence
    • Respond promptly to resident health concerns and emergency situations
    • Maintain accurate and detailed medical records
  • Qualifications

    Qualifications

    • Current LPN license in good standing
    • Valid CPR certification
    • Proven experience in senior care or assisted living environment
    • Strong knowledge of state healthcare regulations and compliance requirements
    • Proficiency in medication administration and treatment procedures
    • Excellent assessment and clinical monitoring skills
    • Strong documentation and reporting abilities
    • Demonstrated ability to work collaboratively in a team environment
    • Excellent communication and interpersonal skills
    • Physical capability to perform essential nursing duties
    • Computer literacy for electronic medical records management
    • Flexibility to work various shifts as needed
    • Ability to maintain resident confidentiality and follow HIPAA guidelines
