    Job Description

    To develop, oversee, and coordinate the Community Outreach Advocacy Program (COAP) for Welvista's Medication Assistance Program (MAP) in a designated region in the state. Facilitate training and serve as the Welvista liaison for healthcare facilities, community organizations, and social service organizations to ensure successful patient enrollment. Manage and implement services aimed at increasing patient enrollment in our program.

    • Serve as primary point of contact for a designated region to facilitate referrals to Welvista programs and services
    • Plan and develop comprehensive community relations and community outreach programs and events to support patient enrollment goals and objectives.
    • Responsible for the administrative and operational tasks associated with generating referrals, completing and approving applications and ensuring compliance with federal and state regulations and Welvista policies.
    • Have complete understanding of Welvista's eligibility requirements and processes.
    • Analyze and report data needed to evaluate Welvista's outcomes assessment practices and procedures. Report regional statistics and performance measurements on deadlines.
    • Provide direct training and education and healthcare providers.
    • Assist in establishing and implementing short- and long-range goals, objectives.
    • Implement strategies for generating referrals for the organization.
    • Must comply with all HIPAA and safety policies, procedures, and regulations.
    • Performs miscellaneous job-related duties as assigned.

    EDUCATION AND EXPERIENCE

    • Bachelor's degree (preferred) or high school diploma or at least 3 years of experience that is directly related to the duties and responsibilities specified; OR at least 7 years of experience that can be demonstrated to be applicable to the duties listed in the job description.
    • Valid South Carolina driver's license, good driving record, reliable vehicle, and proof of auto insurance.
    • Exceptional interpersonal skills and the ability to interact and work effectively with diverse groups of people, including administrators, nonprofit leadership, healthcare organizations, community leadership, public officials, funding agencies, and members of the community at large, including people who may qualify for Welvista medication assistance.
    • Ability to communicate effectively in writing, including email, text computer word processing.
    • Demonstrated leadership, organizational, relationship building, and administrative skills. Report regional statistics and performance measurement skills.
    • Advanced verbal and written communication skills and the ability to present effectively to small and large groups.
    • Ability to self-manage including organizing, prioritizing, and scheduling work.
    • Ability to develop, manage and facilitate referrals to programs.
    • Ability to initiate the application process and follow that application through to completion and approval.
    • Computer proficiency, including intermediate knowledge of Microsoft Outlook, Excel and Word.
    • Basic knowledge of medical terminology.

    WORKING CONDITIONS & PHYSICAL EFFORT

    • Moderate physical activity. Requires handling of average-weight objects up to 25 pounds, standing and/or walking.
    • The physical environment requires the employee to work both inside and outside in heat/cold, wet/humid, and dry/arid conditions.
    • May be occasionally requested to work outside of 8:30am to 5:00pm working hours and weekends for special program events.
    • Work is performed in diverse environments, which may require walking, climbing stairs, carrying materials, setting up tables, and prolonged standing/sitting at exhibitions.
    • Requires extensive travel within the designated region in the State of South Carolina and occasional trips to Welvista's Main Office in Columbia, SC.

    WORK ENVIRONMENT: Work is occasionally performed in an operational office environment. The physical environment requires the employee to work both inside and outside in heat/cold, wet/humid, and dry/arid conditions. Requires extensive travel within the designated region in the State of South Carolina and occasional trips to Welvista's Main Office in Columbia, SC. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
