    The Westchester Institute for Human Development (WIHD), a non-for-profit outpatient clinic in Valhalla, NY is actively recruiting an experienced Registered Dietitian to work a flexible part-time schedule. WIHD provides the broadest array of health services and medical specialties in Westchester and beyond to serve the needs of individuals with developmental disabilities

    Summary of Registered Dietitian Nutritionist (RDN): Delivers person centered care as an integral member of the Adult Health Services multidisciplinary team at WIHD. The Nutritionist provides nutrition management Nutrition management of chronic disease among individuals with genetic and metabolic disorders to include Prader-Willi syndrome, Cystic Fibrosis, Down syndrome, Phenylketonuria (PKU), Fragile X syndrome, Rett syndrome and Williams syndrome.

    In depth nutrition assessment, nutrition support, patient intervention, and documentation of nutrition care in the Electronic Medical Record to include:

    • Conducting patient, caregiver and family interviews to obtain medical history, food/nutrition history, anthropometric data, Body Weight Index (BMI), laboratory values, test results, nutrition focused physical exam.
    • Interpreting results and determining daily estimated nutrient and fluid requirements.
    • Addressing adequacy of appetite/intake and changes in weight (unintentional vs. intentional)
    • Identifying behavioral and social factors in the group home and community that may be affecting dietary intake and identify strategies for meal time support at the group home and in the community.
    • Determining meal and snack patterns; create sample weekly menus to meet special nutrition needs and suggest revisions and product substitutions to meet long term nutritional goals; Utilize vendors such as “My 25” to create individual meal plans within the group residence.
    • Managing acute and long term nutrition consequences of pica, psychogenic polydipsia, food allergies, food sensitivities, food preferences and food aversions.
    • Evaluating bowel regulation, patterns/limitations of physical activity, food availability.
    • Improving accuracy of scale, re-estimating height in contracted individuals. Identifying and managing hypermetabolic conditions, wound healing and states of increased energy expenditure.

    Education and Care Coordination Duties Include :

    • Identifying educational needs of patients, caregivers, family members and providing education at each visit by attending Life Plan Meetings.
    • Counseling effectively to implement and adopt dietary change.
    • Making appropriate referrals (if overdue for APE/bloodwork or in need of other services such as Nephrologist/Endocrinologist etc.) Determining appropriate follow up intervals and following through.

    Administrative and Billing Duties include :

    • Scheduling initial/FU appointments and reschedule appointments upon demand.
    • Create Zoom Meetings and provide tech support to connect with patients, agency staff and family members each day.
    • Determining appropriate procedure and ICD-10 codes and generating superbills.
    • Emailing/faxing Electronic Medical Record Nutrition notes to agency staff Inputting laboratory values and medication changes from outside providers into the Nutrition Flow Sheet.

    Qualifications:

    Education/Licensure: Bachelor’s degree in nutritional science, dietetics or relevant field. NYS Registered Dietitian (RD) from the Commission on Dietetic Registration (CDR)

    Knowledge of: Microsoft Word, Outlook and Electronic Medical Records. Health needs of individuals with developmental disabilities preferred.

    Kind and Length of Experience: At least 5 years experience as a Nutritionist
