As a Rehab and Support Worker, your primary role will involve assisting clients along their path to healing. This may include accompanying them to support sessions, aiding with grocery shopping, or participating in activities that foster their recovery. By working closely with Case Managers and Therapists, you will collaborate on crafting customized rehabilitation plans tailored to address the unique needs of each client. Together, we are dedicated to guiding our clients towards achieving their recovery goals. Responsibilities: • Provide transportation for clients to their appointments. • Facilitate recovery-based group activities. • Assess and evaluate barriers that impact independent or community living. • Address and improve communication skills. • Identify and implement strategies for managing symptoms. • Assist in developing social support systems. • Adapt skills for various areas of life such as home, work, education, leisure, and social activities. • Offer immediate crisis intervention when necessary. • Complete required trainings, both in-person and online. • Maintain detailed documentation on a daily basis. • Meet productivity standards. • Must meet the minimum age requirement of 18 years old for licensing. • Possess a valid driver's license, reliable transportation, and automobile insurance coverage. • Adhere to HIPAA regulations and maintain confidentiality. • Cover shifts at group homes as needed, including day, evening, and overnight shifts. • Schedule and attend necessary meetings. • Demonstrate regular attendance.
