    We are a growing home care agency launching an innovative in-home wound care program designed to deliver high-quality, patient-centered wound management in the comfort of patients’ homes. This program will focus on improving healing outcomes, reducing hospitalizations, and ensuring compliant Medicare billing practices.

    We are seeking an experienced Wound Care Nurse Practitioner to lead, develop, and clinically oversee this new service line.

    Position Summary

    The Wound Care Nurse Practitioner will serve as the clinical lead for our in-home wound care program. This role combines direct patient care with program development, interdisciplinary collaboration, and clinical oversight. The ideal candidate is comfortable working independently in patients’ homes and has strong knowledge of Medicare documentation and billing requirements.

    Key Responsibilities

    Perform comprehensive wound assessments and develop individualized treatment plans

    Provide in-home wound care services, including advanced wound management

    Monitor wound healing progress and adjust care plans as needed

    Collaborate with physicians, home health aides, RNs, and other care team members

    Ensure accurate and compliant clinical documentation for Medicare billing

    Support program development, protocols, and best practices

    Educate patients and caregivers on wound care and prevention

    Participate in quality improvement initiatives related to wound outcomes

    Qualifications

    Active Nurse Practitioner license (APRN) in the state of New Jersey

    National NP certification (AANP or ANCC)

    Wound care experience required (WCC or equivalent certification preferred)

    Home health or home care experience strongly preferred

    Solid understanding of Medicare billing, documentation, and compliance

    Strong clinical judgment and ability to work independently in the field

    Excellent communication and organizational skills

    Valid driver’s license and reliable transportation
