  • Responsibilities

    Job Summary:

    To ensure the delivery of competent, high quality clinical care based on the patients individualized Plan of Care within the LPN scope of practice under the direction of a qualified professional nurse.  

     

    Clinical Assessments :

    • Assist in the completion of the nursing history

    -Delivers age-appropriate care

    -Utilization of critical thinking skills in prioritization of care

     

     

    Plan:

    -Participates in the development & prioritization of an individualized Plan of Care

    -Collaborates with other disciplines in the Plan of Care

    -Assist with discharge planning  needs

    Implementation:

    -Documents interventions that are accurate, pertinent, timely, and legible

    -Administers medications observing the 5 rights

    -Administers medications correctly based on patients age & clinical condition

    -Performs skills & task required by unit expectations

    -Provides a safe environment

    -Includes patient, caregiver, and disciplines in care, council & health teaching

    -Demonstrates correct use & troubleshooting of equipment

    Evaluate:

    -Evaluates the efficacy of medications, treatments and Plan of Care

    -Communicates evaluation to appropriate disciplines

    Professional Attributes:

    -Communicates concerns, assessments to the RN

    -Provides health care at the best value without compromising patient care

    -Responsible & accountable for all actions

    -Utilizes time well

    -Receptive to guidance and direction with informative input

    -Practices within the scope of the license

    Clinical Competence:

    -Implements the Plan of Care as established by the RN Case Manager / Patient/ Physician

    -Adjust the Plan of Care with physician orders as applicable within the LPN scope of practice

    -Promptly reports changes to the RN Case Manager, Patient Care Coordinator or Physician

    -Delivers the Plan of Care, Supplemental physician orders and communication needs in a timely manner

    -Demonstrates proper Infection Control and Safety techniques and knowledge base

    -Demonstrates a knowledge base and practices within the regulatory body requirements of Home Health

    -Demonstrates proper patient/ caregiver teaching skills

    -Participates in the Departments Strategic Quality Management Program

    -Engaged in the growth and development of the Home Health Department

    -Adaptable & Flexible

    Required Skills

    MINIMUM EDUCATION: Graduate from an Accredited School of Nursing

    Required Experience

    MINIMUM WORK EXPERIENCE: Must be able to transport self in inclement weather. Must possess a reliable maintained representative vehicle. At least 1 year experience required. Home Health Experience preferred. Critical Care or Medical / surgical Nursing preferred. Must possess computer / typing skills. Excellent communication skills. Ability to tolerate a high stress environment.
