    Principle Duties and Responsibilities:

    The individual in this role will work under the direct supervision of the licensed professional nurse, to provide safe, effective delivery of patient care. This individual performs a variety of patient care duties and operates a variety of equipment necessary in the care of dialysis patients. This individual provides safe, effective, and compassionate care that is consistent with the State, Federal and Company guidelines, Company policies and procedures, and within his/her scope of practice.

    PRIMARY RESPONSIBILITIES:

    Pre-Treatment responsibilities:

    Prepare clinic for opening shift (where applicable), conduct facility opening procedures such preparing water for treatment, turning on computers, phones etc.

    Prepare hemodialysis equipment per Company policy prior to initiation of patient treatment: (set up and test equipment, priming machine, stringing machine with tubing, checking supplies and stock)

    Treatment Responsibilities:

    Welcome patient and ready patient for treatment

    Assist ambulatory workers in the movement of patient to treatment area

    Initiate and monitor patient treatment; may cannulate patient to begin treatment as directed by the licensed professional and as per State guidelines.

    Under the supervision of the licensed professional nurse, deliver ESRD specific care to assigned patients as prescribed, including all aspects of the hemodialysis procedure.

    Provide routine monitoring of treatment and respond to irregularities or emergencies

    Conduct focused observations of pre and post dialysis treatment

    Monitor key water quality parameters in the absence of the biomedical technician, to ensure safe patient treatment.

    Check and flush patient catheter as needed

    Discontinuation responsibilities:

    Discontinue treatment as directed by the licensed professional and as per State guidelines.

    Provide dressing changes

    Perform final documentation

    Clean the dialysis station (includes chair, machine and ancillary equipment) with the approved facility disinfectant. Ready area for next patient.

    Stock supply carts/cabinets with the supplies necessary to complete the day’s treatments.

    Prepare unit for close (where applicable) including stocking of stations, cleaning and sanitizing of equipment and chairs, finalizing system updates and turning off computers, etc.

    Other Responsibilities:

    Demonstrate effective communication, reporting patient issues as needed throughout the treatment day.

    Under the supervision of the licensed professional nurse, administer and document medications given to the dialysis patients as per physician order and within scope of practice in State. Count Medications and do quality controls.

    Consult the charge nurse when the patient exhibits unusual/abnormal signs and symptoms before, during, and after the dialysis treatment and report unsafe condition immediately

    Perform hemodialysis access care according to policy and procedures.

    Assist the licensed professional nurse in formulating interdisciplinary care plans which include outcomes and timelines for reaching goals, according to established guidelines.

    Participate in CQI projects. Participate in all required meetings including staff, CQI, and educational meetings.

    Perform or assist in the performance of all emergency operational procedures including the evacuation of patients in the event of fire or other disaster.

    Follow Company policies and CDC guidelines for infection control.

    Collect, prepare, and submit ordered patient lab specimens as directed.

    Document all activities related to the patient treatment on the hemodialysis flow sheet, progress notes and electronic record.

    Perform focused assessments under the supervision of an RN

    SECONDARY RESPONSIBILITIES:

    Take on assigned functional management responsibility (such as Anemia Management, Access Management (catheter monitoring), Transplant Management, Foot Care, Bone Management, Case Management or other functional responsibility) and manage program to drive desired outcomes.

    Update information (hospital lists, etc.), collect and send out labs, sign flow sheets, etc.

    Make packs

    Perform monthly summaries

    Utilize opportunities to reinforce education to patients and families.

    Assist with proper handling and storage of hazardous materials.

    Perform all duties in a professional and courteous manner, and maintain confidentiality of all Company and patient records.

    Comply with all Company established policies and procedures.

    Request assistance or advice from supervisor as needed.

    Perform other duties as requested.

    Required Knowledge, Skills and Abilities:

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.     High school graduate or equivalent.

    Graduate of accredited LPN/LVN school and current (active), valid State license.

    Must be certified in IV Therapy

    Dialysis experience preferred, or may complete on-site training course.

    Must have demonstrated record of and commitment to safety; and dedication to excellence.

    Must possess outstanding communication and interpersonal skills. Must be able to communicate professionally and effectively with other staff members, patients and physicians.

    Able to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.

    Must have computer skills including data entry, navigation around software and basic troubleshooting skills.
