    Job Description

    The Social Work Case Manager provides case management and independent living skills training to Transitional Housing Programs participants. This position is responsible for community outreach and program development associated with all Transitional Housing & Youth Development Department programs and assist with the daily operations. 

  • Qualifications

    Qualifications

    • Bachelor's degree in social science, psychology or a related field or equivalent work experience is required
    • Must be 21 years of age or older
    • Bilingual (English/Spanish) preferred
    • Minimum two years’ experience working with young adults (ages 16-24) required
    • Familiar with the operations of a transitional living program
    • Experience with Independent Living Skills issues preferred
    • Ability to work in a very diverse setting with people of all backgrounds, culture and orientation
    • Must have strong personal and professional boundaries to be able to effectively interact with at-risk youth
    • Excellent interpersonal skills with the ability to quickly build rapport with others
    • Must have experience working and communicating within a multidisciplinary team
    • Basic computer knowledge and skills
    • Demonstrate initiative, resourcefulness and problem solving skills
    • Must be able to work independently and as part of a team
    • Ability to work within various deadlines and respond to multiple demands
    • Must be self-aware, flexible, adaptable and teachable
    • Must have reliable transportation to travel throughout San Diego County
    • Must be an adult role model with the ability to effectively set limits and function in crisis-oriented and high stress situations
    • Visual and auditory ability to observe critical incidents and the physical ability to act swiftly in an emergency situation
    • Ability to adequately observe participants’ activities, enforce safety regulations and apply appropriate policies and procedures
    • Must possess the ability to lead and interact in group activities and perform related physical skills
    • Must be available to the program during evening and late night hours, including occasional weekends and holidays
    • Licensing, state law and our funders require that applicants of critical positions within Community Support Services be fingerprinted, prior to reporting to work, and include subsequent arrest notifications
    • Negative TB test results obtained within the past one year, prior to first day
    • Physical exam results obtained prior to first day
    • Pass pre-employment drug testing prior to first day
    • Valid CA Driver’s License with a good driving record and a current CDL H6 print-out (must be dated within 30 days of application) and pass vehicle proficiency exam
    • Current CPR/AED, and First Aid Certification, must include Adult and Child/Infant, and must be from one of the following certifying organizations: American Red Cross, American Heart Association or American Safety & Health Institute

    Additional Information

    • Conduct client-screening interviews and assess service needs via phone and face-to-face interview
    • Assessment and case planning on each participant residing in the Transitional Housing Programs
    • Teach Independent Living Skills and provide support to participants in relational wellness, gaining employment and school opportunities among other areas
    • Provide crises intervention, situational and group counseling to residents and phone clients
    • Assist with program service development and implementation
    • Work with youth on developing social skills, communication, higher order thinking skills, self-control and positive self-concept
    • Coordinate case planning with residents, clients and collaborating agencies
    • Provide advocacy and referrals based on clients’ needs
    • Communicate effectively with program leadership, co-workers, participants, families and other service providers
    • Ensure that YMCA policies & procedures and applicable laws are adhered to at all times
    • Work independently and as a team member to create innovative ways to meet contract goals and provide a high level of service that is engaging and meets the needs of the participants
    • Keep well-written; concise case notes to include all pertinent information for each participant using required tracking systems
    • Maintain organized records and case files for all participants
    • Transport participants in YMCA vehicle to and from activities and appointments as needed
    • Practice non-aggressive defensive driving techniques
    • Operate YMCA vehicles in safe manner
    • Perform other administrative tasks as assigned and assist with maintaining proper document storage and auditing
    • Participate in and attend all required staff meetings, trainings, YMCA staff development events, and appropriate agency-wide committees
    • Ability to work effectively with others in alignment with the YMCA 4 Core Values
