    Our Ideal Candidate: Requires great flexibility and team-oriented approaches. This exciting position provides full scope counseling services to support a diverse population and coordinate care for multi-issue clients.

    Some technical requirements for this position are:

    • Diagnose and treat substance use disorders.
    • Create, recommend, and document treatment plans.
    • Provide counseling sessions (individual or group), using methods as directed.
    • Take action on the measures required to support our diverse population.
    • Coordinate the care of multi-issue clients and intervention with their families.
    • Working with a team of health professionals and other support staff, do intervention for identified risk factors through the use of developed care plans.
    • Provide behavioral health screens and primary care behavioral health for patients.
    • Develop and maintain appropriate linkages with other mental health/social services in the community who share patients with us.
    • Using current knowledge of community resources, expedite referrals.
    • Work in coordination with other YNHS health providers to develop education programs for patients and staff.
    • Coordinate with social services personnel to address the complex needs of individuals and families, both at our main clinic and at a satellite location, and work in partnership with the other disciplines to meet additional health-affecting conditions.
    • Maintain documentation on all case files including patient information, treatment plan, goals, progress and outcomes using EMR.
    • Maintains patient confidentiality.
    • Participate in quality assurance activities and peer review as requested.
    • Maintain organized, clean, efficient and confidential work area.
    • Represent YNHS in local meetings as requested.
    • Perform related work and other tasks as requested.

    Overview:

    Requires sitting for long periods of time, working at a computer terminal. Some bending and stretching required. Ability to lift papers or boxes up to 30 pounds occasionally. Requires corrected vision and hearing to normal range. Manual dexterity and eye-hand coordination required for use of calculators, phones, computer keyboards and other equipment. Requires ability to sustain concentration in a noisy and fast-paced environment. Visitation in client homes year-round is common. Local travel using own car is required.
