Summary
The Service Integration Specialist is responsible for coordinating efforts and activities of Yamhill County’s Service Integration program and developing partnerships to expand services to individuals and families in need. Core areas of focus include building partnership with cross-sector partners and service providers, providing facilitation and coordination of Service Integration Teams (SIT), developing, and maintaining communication and collaboration among all key partners, developing community resources, and providing leadership among community partners by performing the following duties.
Essential Duties
- Assists with and supports the development of strategies and goals as identified by the local SITs and the Early Learning Council.
- Develops, supports, and maintains collaboration and promotes healthy, viable collaborations in both the public and private sector.
- Develops and coordinates SIT projects, activities and events, including community outreach events.
Job Duties
Service Integration Teams
- Coordinates with and provides support to Service Integration Teams.
- Coordinates and assists in alignment with healthcare transformation, strategic, and early learning goals.
- Facilitates SIT meetings.
- Maintains a working relationship with key partners and serves on committees and work groups, as appropriate.
- Builds team meeting agendas and engages community partners to participate and present.
- Monitors the team’s funds and processes funding requests, including emergency requests.
- Coordinates partner training and provides technical assistance for staff, service providers and community partners.
- Works collaboratively with state and county agencies, non-profit organizations, cultural and educational institutions, community-based organizations, and the faith community to develop and maintain access to support programs and services.
- Conducts outreach to agencies to promote participation in SIT.
- Prepares reports as needed for Yamhill Community Care YCCO governance and advisory councils, SIT partners, and the Department of Early Learning & Care (DELC), and prepares reports, newsletters and other promotional materials as needed for partners, community members and funders.
- Generates resources for continuation and expansion of Service Integration projects.
- Coordinates SIT member training and the development of strategies and goals as identified by the local SI teams.
- Maintains regular communication with SIT members regarding funding requests and awards.
- Tracks data and writes grants and measures outcomes of the program.
- Prepares MOU’s and invoices.
- Ensures confidentiality practices meet required mandates.
Community Engagement
- Works collaboratively with YCCO leadership to establish community engagement priorities, timelines and budget that enhance the access to and delivery of early learning, family support and health services to YCCO members.
- Utilizes innovative approaches to identify and engage YCCO members, community leaders, advocacy groups, advisory groups, and aligned organizations.
- Designs presentations and supporting materials for community outreach activities and meets with individual stakeholders and organizations to communicate YCCO’s goals and activities and increase knowledge of community engagement activities.
- Builds partnerships with cross-sector partners to increase funding available to assist individuals and families in need.
- Builds partnership with vendors to facilitate the provision of timely, effective, and cost-efficient services to individuals and families in need.
- Participates in community outreach events to promote YCCO goals and resources for health, school readiness and family support.
- Serves as a community resource and ensure the highest level of customer service to YCCO members, community organizations, committees and the DELC functions.
- Represents YCCO internally and with external organizations.
Community Advisory Council
- Attends monthly Community Advisory Council (CAC) meetings.
- Works closely with YCCO Community Health Specialist to support recruitment for the CAC and integration of Early Learning into the Community Health Improvement Plan (CHIP).
Other Duties
- Works closely with Outreach & Referral Specialist to connect families of young children to support services.
- Actively participates in weekly Early Learning team meetings and monthly YCCO staff meetings.
- Supports Early Learning team events, as needed.
- Participates in work review and coaching plans, as needed.
- Participates in collaborative learning opportunities locally and at the state level.
Essential Department & Organizational Functions

- Participates in the preparation and submission of regulatory and contract required deliverables.
- Works closely with other YCCO departments, including Health Plan Operations and Compliance to assist with audits; including the External Quality Review (EQR), as needed.

Education & Experience
Required:
- Bachelor’s degree in a major related to social services (psychology, sociology, etc.) and
- Two (2) years of experience in developing community resources and collaborative projects, team building, and developing of managing community groups, or non-profit; or
- any equivalent combination of education, experience, and training, required.
- Minimum One (1) year experience working with at-risk populations.
- Minimum One (1)year experience performing office, administrative, or coordination tasks.
OR:
- Any combination of education and experience that would qualify candidate for the position.
Preferred:
- Bilingual English/Spanish.
