    +--------------------------------------------------------------------------+ | SUMMARY | | | | The Outreach & Referral Coordinator is responsible for providing | | outreach and referral coordination to local individuals, agency partners | | and the community as a whole regarding health plan and social services, | | processes, and access. This position coordinates and assists in | | alignment with healthcare transformation, strategic, and early learning | | goals by performing the following duties. | | | |   | | | | ESSENTIAL DUTIES | | | | - Assists in developing outreach plans for underserved children and | | families, in collaboration with the Early Learning Program Manager | | (ELPM), the Family C0RE Leadership Team and the Early Learning | | Council and plays an active role in implementation and | | ongoing administration. | | - Works with Community & Network Relations Specialist to develop | | promotional material and to integrate messaging into social media. | | - Creates networking and professional development opportunities to | | support the home visiting network. | +--------------------------------------------------------------------------+

    EXPERIENCE & EDUCATION:

    Required:

    • Bachelor’s degree in social work, public health, or related field or Associate degree in early childhood education with 5 years’ of experience in early childhood.
    • Minimum one year experience performing administrative, data tracking, and coordination tasks. 
    • Minimum one year experience working with at-risk populations. 
    • Bilingual (Spanish/English).

     

    Preferred:

    •  Experience in care coordination and/or home visiting.
