Ranked nationally two years in a row as a “Best Places to Work” by Modern Healthcare, this growing northern California nonprofit healthcare organization is seeking an experienced and innovative Admissions RN who is squarely focused on collaboration, relationship management, and education. YoloCares is Joint Commission accredited and the only organization in the Sacramento region to be accepted into the esteemed National Partnership for Hospice Innovation.

We are looking for a dedicated Admissions – Registered Nurse to assist patients and families in understanding the YoloCares program of care, conducts an initial assessment, and evaluates the immediate needs of the individual. In this position, you are an essential member of the interdisciplinary team who provides professional care as prescribed. You will be responsible for completing nursing assessments, collaborate with other core disciplines, schedule nursing visits, and other duties as assigned.

This position is M-F and can be either 10:00am-6:30pm or 11:00am-7:30pm

JOB QUALIFICATIONS:

· A current license in the State of California as a Registered Nurse.

· Bachelor’s degree in Nursing from an accredited school.

· Minimum of one (1) year acute care/hospice experience preferred.

· Must be knowledgeable about Hospice Medicare Conditions of Participation and California Hospice Licensing Standards.

· Excellent communication skills, both verbal and written, are required.

· Ability to communicate well in English (spoken and written) is required. Bilingual in Spanish, preferred.

· Ability to handle frequent interruptions and to complete duties in a timely manner.

· Ability to successfully meet agency background screening requirements.
