We are currently seeking passionate and dedicated Case Management Specialists to join our Mobile Treatment Services (MTS) and Psychiatric Rehabilitation Program (PRP) in Prince George’s County. MTS and PRP are intensive, community-based programs that provide personalized support to individuals of all ages with mental illness. Our program offers treatment and support to those who may be homeless, struggling with substance abuse or trauma, and/or have not responded well to traditional outpatient treatment. Services are provided in the patient’s home, street, shelter, or anywhere else in the community.
As a Case Management Specialist for MTS and PRP, you will be a vital member of our multidisciplinary team, collaborating with medical prescribers and clinicians to provide comprehensive mental health care to patients out in the community. MTS and PRP serve a diverse population with a wide range of mental health conditions, including schizophrenia, mood, and bipolar disorders. As a member of the treatment team, you will play a critical role in our mission to improve lives in our community.
RESPONSIBILITIES
Ensure compliance with all federal, state, and iMind policies and procedures.
Thoroughly complete all iMind, State and County documentation in a timely manner.
Accompany the clinician to provide services in the community and assist with appropriate clinical work, including referrals to internal and external resources as needed.
Transport clients to other appointments as needed.
Attend all staff meetings and actively participate by addressing potential crises, problematic cases, general case reporting, and resource information.
Represent iMind in a professional manner, educate the community about services, and foster collaborative relationships.
Take responsibility for professional growth by attending all mandatory iMind trainings and staying up to date with industry expectations.
Provide outreach, advocacy, and other needed services to clients in any environment including: the streets, shelters, apartments, office, etc.
Inspire and motivate patients through teaching and role modeling the value of every individual's recovery experience.
Share your personal insights and perspectives on the recovery journey and offer support and guidance to patients who may be facing similar challenges.
Advocate for the importance of recovery-oriented care, and work to reduce stigma and promote awareness of mental health issues in our community.
Assist in developing and modeling effective coping techniques and self-help strategies.
Maintain written and digital records, compile reports, and complete other necessary program documentations (i.e., case notes, statistics, letters, etc.).
Assist patients in managing their finances, including preparing budgets and computing expenses.
Educate, inform, and advocate for clients regarding benefits and entitlements (Social Security, Public Assistance, food stamps, etc.).
Perform other duties as assigned to support the team and organization.
QUALIFICATIONS
Maryland Certified Peer Recovery Specialist (CPRS) credential preferred.
Associate or bachelor’s degree in a mental health related field or documented 1-3 years of experience in human services.
Bilingual (English/Spanish) is preferred but not required
Must have a clean driving record and reliable transportation
Must be willing to complete Criminal Background Check
