    Evergreen Home for Special Care is a privately owned continuous care provider located in the beautiful Annapolis Valley.

    The facility is comprised of a Children Centre which is licensed to provide 24-hour nursing coverage for 20 children and a Seniors Centre consisting of 98 residents. All residents receive multi-disciplinary care based on a holistic approach to ensure physical, mental and spiritual wellbeing while recognizing the right to individuality, respect and dignity.

    Programming in the Children Centre is conducted on an individual basis and some of the children attend school outside the facility. Full and part time professional nursing care is provided to children of all ages and respite care is available to families in need.

    Key Responsibilities:

    The Social Worker - Children Centre is responsible to the Director of Nursing for the Children Centre, and have the opportunity to promote the social work profession in long term care by providing expertise in bio-psychosocial and person in environment assessments, individual and family counseling, and group support on topics such as caring for caregivers, grief, and loss.

    Qualifications:

    • Completion of a minimum of bachelor‚Äôs degree in social work
    • Completion of Master of Social Work or current enrollment in masters program is an asset.
    • Must be a Registered Social Worker in Nova Scotia or eligible for registration in the province.
    • Must have at least two years of social work professional experience.
    • Resident centered care as the core motivator for service delivery.
    • Be well versed in dealing with people with various backgrounds and personalities.
    • Strong understanding and advocation for best practices in healthcare.
    • Strong computer and analytical skills.
    • Must have the ability to deal with complex situations and make sound recommendations.
    • Ability to network and develop strong relationships with team members and community partners.
    • Strong oral and written communication skills.
    • Strong conflict resolution skills.

    Hours of Work:

    • FTE 1.0
    • 8 hour shifts
